摘要
目的:探讨消渴病(2型糖尿病)中医辨证分型特点,分析血脂与证型的相关性,为消渴病(2型糖尿病)中医辨证提供客观依据。方法:收集200例合格2型糖尿病病例,按阴虚热盛证、湿热困脾证、气阴两虚证、阴阳两虚证,血瘀脉络证辨证分型,同时配以50例健康体检者做对照。分析不同证型血脂代谢的关系。结果:气阴两虚最多见;气阴两虚和阴阳两虚证型患者的血脂除了低密度脂蛋白(LDL)外,均与对照差异明显,均有统计学意义(P<0.05);血瘀脉络证患者的各血脂类(TC、TG、HDL、LDL)与阴虚热盛、湿热困脾、气阴两虚和阴阳两虚证型比较,均差异明显,有统计学意义(P<0.05)。结论:消渴病(2型糖尿病)辨证分型以气阴两虚型最多,证型变化有从阴虚热盛、湿热困脾、气阴两虚到阴阳两虚的趋势;2型糖尿病中医辨证分型与血脂间存在相关性,对中医辨证客观化具有参考价值。
Objective:To discuss the characteristics of TCM syndrome classification of type 2 diabetes ,analyze the relationship between blood lipid and syndrome types ,and provide the objective evidences for syndrome differentiation. Methods:The cases of type 2 diabetes(n=200)were collected and divided into groups of yin deficiency and heat excess,dual deficiency of qi and yin ,dual deficiency of yin and yang,dampness-heat encumbering spleen and blood stasis.Other 50 healthy cases were chosen as control group.The relationship between syndrome types and blood lipid metabolism was analyzed.Results:The most common type was dual Deficiency of qi and yin.There was statistical difference in all lipid (TC ,TG ,HDL ,LDL ) except low-density lipoprotein(LDL) between dual deficiency of qi and yin,dual deficiency of yin and yang and control group(P〈0.05).The differences in all lipid (TC, TG, HDL, LDL) had statistical significance(P〈0.05)between group of blood stasis and other groups (yin deficiency and heat excess,dampness-heat encumbering spleen,dual deficiency of qi and yin,dual deficiency of yin and yang). Conclusion:The most common syn&'ome type of type 2 diabetes is dual deficiency ofqi and yin, and the changes have a tendency from yin deficiency and heat excess to dampness- heat encumbering ,spleen to dual deficiency ofqi and yin to dual deficiency ofyin and yang.There is a correlation between syndrome types and blood lipid ,which has reference value for the objectification of TCM syndrome differentiation.
