超声引导下腹横肌平面阻滞麻醉在胃癌手术中的镇痛效果观察

Analgesic effect observation of ultrasound-guided transversus abdominis plane regional anesthesia on treating patients with gastric cancer surgery

目的探讨超声引导下腹横肌平面阻滞麻醉在胃癌手术中的镇痛效果。方法选取胃癌患者40例,随机分为2组,观察组采用超声引导下腹横肌平面阻滞,对照组实施常规麻醉。对比2组术后镇痛效果、心率、平均动脉压变化情况及舒芬太尼药物使用量。结果观察组拔管后及术后各时间点VAS评分均低于对照组(P<0.05);切皮时,对照组心率及平均动脉压均较切皮前显著升高,而观察组切皮后心率及平均动脉压水平显著低于对照组,舒芬太尼使用量显著少于对照组(P<0.05);2组患者围术期均未发生明显不良反应。结论采用超声引导下腹横肌平面阻滞有利于缓解胃癌根治术患者的术后疼痛感。

Objective To investigate the analgesic effect of ultrasound-guided transversus abdominis plane regional anesthesia on treating patients with gastric eancersurgery. Methods A total of 40 patients with radical operation of gastric cancer were randomly divided into two groups. The ob- servation group was treated with ultrasound-guided transversus abdomins plane regional anesthesia, while the control group was treated with routine anesthesia. Postoperative analgesic effect, heart rate, mean arterial pressure and sufentanil dose were compared between two groups. Results At the time points of after extubation and each time points after operation, the VAS scores in the observation group were significantly lower than the control group （P 〈 0.05 ）. At the time point of skin incision, the heart rate and mean arterial pressure in the control group significantly increased when compared with that before skin incision, and the heart rate and mean arterial pressure after skin incision in the observation group were significantly lower than the control group, and sufentanil consumption was sig- nificantly less than the control group （ P 〈 0.05 ）. There were no obvious adverse reactions in both groups. Conclusion Application of ultrasound-guided transversus abdominis plane regional anesthesia can alleviate postoperative pain in patients with radical resection of gastric cancer.
