探讨腹部创伤的螺旋CT检查及临床意义

To Investigate the Spiral CT Examination and Clinical Significance of Abdominal Trauma

目的:探讨多层螺旋CT在急诊腹部创伤诊断中的应用价值及临床意义。方法:选取2014年1月-2017年1月笔者所在医院急诊收治的腹部创伤患者180例作为研究对象。所有患者经急诊收治入院后均接受腹部超声检查及多层螺旋CT平扫检查,对比两种检查方式对腹部创伤的诊断结果,并与多次复查结果或手术探查结果进行比较,分析多层螺旋CT在急诊腹部创伤诊断中的应用价值。结果:螺旋CT对腹部脏器创伤诊断的敏感性为96.5%,特异性为95.5%,准确率为96.1%;超声与CT诊断临床符合率、脏器损伤程度分级诊断的临床符合率比较,差异均无统计学意义(P>0.05)。结论:螺旋CT平扫对腹部创伤导致的脏器损伤敏感性较高,能够快速、准确诊断疾病,并可提供具有参考价值的指导根据,具有临床应用及推广价值。

Objective：To explore the value and clinical significance of multi-slice spiral CT in the diagnosis of emergency abdominal trauma.Method：A total of 180 patients with abdominal trauma from January 2014 to January 2017 were selected as the study subjects.All patients underwent abdominal ultrasound examination and multislice CT scan after emergency admitted,compared the two types of inspection results with the diagnosis of abdominal trauma and the result of multiple reexamination or surgical exploration.The multislice CT in the diagnosis of abdominal trauma emergency application value was analyzed.Result：The sensitivity,specificity,accuracy of spiral CT to abdominal viscera trauma were 96.5%,95.5%,96.1%,there were no significant differences of diagnosis,and the clinical coincidence rate of grading diagnosis of viscera injury between ultrasonography and CT（P0.05）.Conclusion：Spiral CT scan of abdomen viscera lesions caused by trauma has high sensitivity,quick and accurate diagnosis of diseases,and can provide a reference value,according to the guidance of clinical application and popularization value.
