术前导尿对经会阴前列腺穿刺活检术中尿道损伤的影响

Effect of preoperative catheterization on urethral injury in transperineal prostate biopsy

目的 探讨术前导尿对减少经会阴前列腺穿刺活检术中尿道损伤的应用价值.方法 62例拟行经会阴前列腺穿刺活检术的患者随机分为观察组和对照组,观察组32例给予术前导尿,对照组30例不留置导尿.两组患者的穿刺方法相同.比较两组患者的穿刺阳性率和术后血尿、排尿困难、尿潴留的发生率.结果 两组患者的穿刺阳性率差异无统计学意义（P＞0.05）,观察组患者的血尿、排尿困难、尿潴留的发生率分别为28.1％（9/32）、12.5％ （4/32）、6.25％ （1232）,均显著低于对照组的56.7％ （17/30）、36.7％ （11/30）、30.0％ （9/30）,差异具有统计学意义（P＜0.05）.结论 术前导尿不影响经会阴前列腺穿刺活检术的穿刺质量,有助于减少尿道损伤,值得临床推广应用.

Objective To evaluate the value of preoperative catheterization in reducing urethral injury in patients undergoing transperineal prostate biopsy.Methods Sixty-two patients who underwent transperineal prostate biopsy were randomly divided into observation group and control group,and 32 cases in observation group were given preoperative catheterization,while 30 cases in control group were without catheterization.The positive rate of prostate biopsy and postoperative hematuria,dysuria and urinary retention were compared between the two groups.Results There was no significant difference between the two groups in the positive rate of prostate biopsy （P ＞ 0.05）.The incidence of hematuria,dysuria and urinary retention in the observation group was 28.1％ （9/32）,12.5％ （4/32） and 6.25％ （2/32）,respectively,and which were significantly lower than those in the control group 56.7％ （17/30）,36.7％ （11/30）,30.0％ （9/30）,respectively,the differences were statistically significant （P ＜ 0.05）.Conclusions Preoperative catheterization in transperineal prostate biopsy will not affect the prostate biopsy quality,and it is helpful to reduce urethral injury.It is worthy of popularization and application.
