基于红外热图像和热程的降温与舒适度研究

摘要 目的研究热水擦浴与热水泡足两种操作对发热患儿体表皮肤温度及体温和舒适度的影响。方法选取65例发热患儿,按体温热程分为体温上升期和体温下降期,按日期分为泡足组(45例)和擦浴组(20例)。在操作前后3次重复测量腋温、肛温和皮温(FLIR红外热像仪摄图采温),同时利用Wong-Baker面部表情量表评价操作前、中、后的舒适度。结果在上升期,热水泡足短时间内使腋温升高、皮温升高;在下降期,使各温下降曲线更平缓。热水擦浴则相反,两者差异只有上升期的面温有统计学意义(P=0.019)。操作有效时间内,舒适度上升期热水泡足组最高,下降期热水擦浴组其次,上升期热水擦浴组最低。结论发热时物理方法对降温无临床意义,对皮温和舒适度有影响。 Objective To study the effects of two kinds of hot water bath and hot water foam on the body surface temperature and body temperature and comfort of children with fever. Methods 65 cases of children with fever were selected,according to the degree of temperature,it is divided into the period of temperature rise and the period of hypothermia. According to the date,it was divided into follicular group( 45 cases) and bathing group( 20 cases). The axillary temperature, Anal temperature and skin temperature( FLIR infrared thermography) were repeated 3 times before and after operation. Meanwhile,the Wong-Baker facial expression scale was used to evaluate the comfort level before,in and after operation.Results In the rising period,hot water foot short time to make the axillary temperature rise,skin temperature increased; in the declining period,the temperature drop curve more smooth. The hot water bath was the opposite,and the difference was statistically significant only in the rise period( P = 0.019). In the effective time of operation,the hot-water soaking group was the highest in the rise period of comfort,the second of the descending hot water bath group,and the lowest in the rising hot water bath group. Conclusions During fever period,the physical method had no clinical effect on cooling,had effect on the skin temperature and comfort.
