运用张锡纯宗气理论治疗重症肌无力经验

Experience on treating myasthenia gravis by using pectoral qi theory of ZHANG Xi-chun

重症肌无力是一种由神经-肌肉接头处传递功能障碍所引起的自身免疫性疾病,属中医痿证范畴。笔者认为重症肌无力病位在肌肉及血脉,病因与宗气相关,患者或素体脾虚、肾虚或感受热毒耗伤肺气致宗气亏虚,或感受湿热,或饮食所伤内生湿热、痰湿等致宗气瘀滞,总属宗气亏虚或宗气瘀滞不能撑持全身,神气不达,不能主司动作所致,病性多虚实夹杂,治当以调补宗气。宗气得补,邪气得除,故能撑持全身;气血周流,神气达于肌肉,故能主司动作,重症肌无力症状可以得到根本缓解。

Myasthenia gravis（MG） is an autoimmune disease caused by neuro-muscular junction transmission dysfunction, and it belongs to the category of flaccidity syndrome in TCM. The author believes that disease location of MG is located in muscles and blood vessels, and the pathogenesis is associated with pectoral qi. Or the patients are pectoral qi deficiency, who are born with spleen deficiency or kidney deficiency or infection external heat-toxicity damaged lung qi. Or the patients are pectoral qi stagnation, caused by infection external damp-heat, and damp-heat and phlegm-damp caused by internal injury diet. In a word, pectoral qi deficiency or stagnation cannot support the whole body, and vital essence cannot spread the whole body and cannot govern movement. The nature of the disease is excess and deficiency, and adjusting and benefiting pectoral qi should be a therapeutic principle. The adjustment means excluding the obstacles of the movement of pectoral qi, such as blood stasis, phlegm and retained fluid, and other exogenous evil in the internal organs or channels and collaterals. Benefiting pectoral qi concludes in benefiting lung qi, spleen qi and vitality. The pectoral qi is benefited, pathogenic qi is got rid of, and then the man＇s whole body is supported. Qi and blood circulating in the bodies, vital essence nourishing muscles and governing the movement, then the symptoms of myasthenia gravis can be fundamentally relieved.
