系统性康复锻炼对膝关节骨性关节炎行关节镜手术后患者关节功能的影响及疗效观察

Effect of systemic rehabilitation exercise on joint function of patients with knee joint osteoarthritis after arthroscopic surgery and its curative effect

目的探讨系统性康复锻炼对膝关节骨性关节炎(OA)行关节镜手术后患者关节功能的影响及疗效观察。方法选取2016年1~12月骨科住院手术治疗的膝关节OA患者74例,随机将其分为康复组和对照组。两组患者予以行关节镜手术,对照组患者术后予以常规康复训练,康复组患者在对照组基础上予以系统性康复锻炼干预。观察并比较两组术前及术后6个月膝关节疼痛和功能的变化,并评估其临床效果。结果术后6个月,两组VAS评分较前均有不同程度下降(P<0.05或P<0.01),且康复组下降幅度比对照组更显著(P<0.05);同时康复组膝关节Lysholm评分优良率高于对照组(χ~2=5.10,P<0.05),临床总有效率优于对照组(χ~2=4.16,P<0.05)。结论系统性康复锻炼干预可提高膝关节OA行关节镜手术后的临床效果,能促进术后关节功能康复,减轻其疼痛程度,提高其活动度,改善关节功能。

Objective To investigate the effect of systemic rehabilitation exercise on the joint function of patients with knee osteoarthfitis（OA） after arthroscopic surgery and its curative effect. Methods A total of 74 patients with knee OA who were treated with surgery in the Department of Orthopedics fiom January to December 2016 were randomly divided into rehabilitation group and control group. The patients in the two groups were treated with arthroseopic surgery. The patients in the control group were treated with routine rehabilitation training after surgery. The patients in the rehabili- tation group were treated with systemic rehabilitation on the basis of the treatment of the control group. The changes of knee joint pain and function in the two groups were observed before and 6 months after operation, and the clinical curative effect was compared. Results The VAS scores of the two groups at 6 months after surgery were significantly lower than those before surgery（P〈0.05 or P〈0.01）, and the descending rate of the rehabilitation group was more obvious than that of the control group（P〈0.05）. The excellent and good rate of the knee Lysholm score in therehabilitation group was significantly higher than that of the control group（x2=5.10,P〈0.05）. The total effective rate of rehabilitation group was significantly better than the control group（x2=4.16,P〈0.05）. Conclusion The systematic rehabilitation exercise can improve the clinical effect of knee joint OA after arthroscopy surgery, which can promote the rehabilitation of postoperative joint function, improve postoperative pain, joint mobility and knee joint function.
