宁夏2型糖尿病住院患者慢性并发症的现状调查及影响因素分析

Chronic complications of type 2 diabetes mellitus inpatients in Ningxia and its influence factors

目的调查2型糖尿病住院患者慢性并发症的发生情况并分析影响因素,为综合防治提供参考依据。方法回顾性调查分析2015年1-12月在宁夏某三甲医院内分泌科住院的2型糖尿病患者病案信息,了解其慢性并发症的发生发展情况。结果 739例患者中,慢性并发症总发生率为74.6%,发生频率排名前5依次为大血管病变、肾脏病变、视网膜病变、周围血管病变和神经病变。多因素Logistic回归结果显示,病程(OR=1.004,95%CI:1.002~1.006)和职业(以工人为对照,农民OR=3.122,95%CI:1.420~6.861;无职业人员OR=2.771,95%CI:1.389~5.527)是2型糖尿病慢性并发症发生的独立危险因素。结论2型糖尿病慢性并发症的总发生率较高,防治形势不容乐观。故应加强对可控危险因素的综合干预,减少并发症的发生,从而降低医疗费用,提高患者整体生存质量。

Objective To investigate the incidence and influence factors of chronic complications of inpatients with type 2 diabetes mellitus （T2DM） and to provide reference for the comprehensive prevention and treatment. Methods A retrospective analysis was conducted to analyze the occurrence and development of chronic complications of T2DM hospitalized patients in a third-level first-class hospital of Ningxia from January to December in 2015. Results All of the 739 T2DM patients, the total incidence of chronic complications was 74.6%. The top five chronic diseases were large vessel disease, renal disease, retinopathy, peripheral vascular disease and neuropathy. Multivariate logistic regression analysis showed that the duration of disease （OR = 1.004, 95% CI： 1.002 - 1.006） and occupation （taking workers as controls, farmers＇ OR = 3.122,95%CI： 1.420 - 6.861, unemployed persons＇ OR = 2.771,95%CI： 1.389 - 5.527） were the independent risk factors of chronic complications of T2DM. Conclusions The total incidence of chronic complications of T2DM is high. We should strengthen the comprehensive intervention of controllable risk factors for reducing the incidence of complications, decreasing medical costs and enhancing overall quality of life of patients.
