临床药师对心功能不全合并地高辛中毒患者的药学监护

Pharmaceutical Care for One Patient with Heart Failure Complicated with Digoxin Poisoning by Clinical Pharmacists

目的:探讨临床药师对心功能不全合并地高辛中毒患者的药学监护过程,探讨问题和对策,促进合理用药。方法:回顾性分析心功能不全合并地高辛中毒患者的治疗及药学监护过程。结果:地高辛中毒可通过监测血药浓度明确诊断,临床药师积极发挥专业优势,协助临床医师制订和完善了治疗方案,给予患者停药、对症处理等措施后,患者病情好转出院。结论:地高辛治疗窗窄且存在明显个体差异,血清浓度影响因素复杂,故应根据患者病情给予个体化治疗方案,同时应加强对血药浓度、肾功能及电解质的监测,降低地高辛中毒的发生率,促进临床合理用药。

OBJECTIVE： To probe into the pharmaceutical care process for one patient with heart failure complicated with digoxin poisoning by clinical pharmacists,and to investigate the problems and countermeasures,so as to promote the rational drug use. METHODS： Retrospective analysis was conducted on the pharmaceutical care process for one patient with heart failure complicated with digoxin poisoning by clinical pharmacists. RESULTS： Through the monitoring of blood concentration,patient with digoxin poisoning could be found immediately. The clinical pharmacists took their advantages actively and assisted the clinicians to formulate and improve the treatment regimen; after being given measures of withdraw and symptomatic treatment,the patient was discharged from hospital. CONCLUSIONS：The treatment of digoxin poisoning is limited and has obvious individual difference,and the effect factors of serum concentration are complex,therefore,it is necessary to provide individualized treatment regimens according to patient＇s conditions,strengthen the monitoring of blood drug concentration,renal function and electrolyte and reduce the incidence of digoxin poisoning,so as to promote the rational use of drugs.
