上颌窦后部气化程度对鼻腔形态影响的CT观察 被引量:2

Effect of the pneumatization of the posterior maxillary sinus on nasal cavity morphology:basing on CT observation

摘要 目的探讨上颌窦后部的气化程度对鼻腔形态的影响。方法选择2012年5~12月在我院门诊行鼻窦CT检查的103例患者进行研究,无头颅及鼻窦外伤、手术及肿瘤史。观察其上颌窦后内侧上部的气化及其鼻腔形态的影响。结果 (1)上颌窦后部内侧壁无内移者占48.5%(50/103),内移者占51.5%(53/103)。(2)上颌窦内侧壁的内移以中鼻甲根气化内移为主占75.5%(40/53)以下鼻甲根部气化内移为主占9.4%(5/53),上颌窦内侧壁中部(M段)整体出现内移占15.1%(8/53)。(3)上颌窦内侧壁无内移组中鼻甲根间距平均为(29.55±3.71)mm,显著宽于内移组(23.65±3.64)mm,但下鼻甲根间距在两组之间并无显著差异。结论上颌窦内侧壁后部内移会导致鼻腔后部宽度变窄,可能是通气阻力增加的原因之一。 Objective To observe the pneumatization of the posterior maxillary sinus (MS) and its effect on nasal cavity morphology. Methods One hundred and three outpatients encountered in our hospital during Ma2012 to December 2012 were included in this study. Patients were required to have no sinus trauma, surgery, and tumor histo- ry, whose paranasal sinus computed tomography scans were analyzed from scheduled axial and coronal plane. Results In 48.5% (50/103) cases, the medial wall of the maxillary sinus (MMS) had no shift toward the medial, and in 51.5% (53/103) of the cases, the MMS had a shift toward the medial. Among the cases of the MMS shift toward the medial, 75.5% (40/53) of the cases were mainly extends toward the middle turbinate (MT) . In 9.4% (5/53) cases, the pneumatization of MS were mainly extends toward the inferior turbinate (IT) . In 15.1% (8/53) cases, the middle part of the MMS (M segment) showed an overall shift toward the medial. The mean distance between the mid- dle turbinate was (29.55±3.71 ) mm in the group of the MMS without shifts toward the medial, which was significantly larger than that in the group of the MMS shifts toward the medial (23.65±3.64) mm (P〈0.05) . However, there was no significant difference between the two groups in the distance between the inferior turbinate roots. Conclusions The excessive pneumatization of MS would make the upper part of MMS shift toward medial obviously, leading to the narrowing of the upper part of posterior nasal cavity. It may be the cause of increased ventilation resistance in patients.
