功能性内镜鼻窦手术对慢性鼻-鼻窦炎患者嗅觉功能的影响

Effect of functional endoscopic sinus surgery on olfactory function in patients with chronic rhinosinusitis

目的探讨功能性内镜鼻窦手术对慢性鼻-鼻窦炎(CRS)患者嗅觉功能的影响。方法选择2016年5月~2017年5月我院收治的40例CRS伴嗅觉障碍的患者作为研究对象,所有患者均行功能性内镜鼻窦手术(FESS),采用T&T嗅觉计定量检查法检测术前术后嗅觉功能,比较术前及术后第4、12及24周的嗅觉功能恢复情况。结果术前嗅觉功能轻度减退者10例,中度减退者14例,重度减退者4例,嗅觉丧失者12例,术后4周、12周、术后24周后,嗅觉功能逐渐改善,尤其是24周时,嗅觉正常者25例,轻度减退者6例,中度减退者4例,重度减退者1例,嗅觉丧失者4例,所有患者术后嗅觉功能均较术前发生显著变化(P<0.05);嗅觉T&T测试显示,术后4周,有30.0%患者嗅觉改善,术后12周,有60.0%患者嗅觉改善,术后24周,有72.5%的患者嗅觉改善。结论 CRS伴嗅觉障碍患者接受FESS治疗后,嗅觉会得到改善,这对于临床治疗工作具有十分重要意义,值得关注并推广。

Objective To investigate the effects of functional endoscopic sinus surgery on olfactory function of patients with chronic rhinosinusitis. Methods From May 2016 to May 2017, 40 patients who had olfactory dysfunction and chronic nasal sinusitis in our hospital as the research object, all patients were performed endoscopic sinus surgery, using the T＆T olfactometer quantitative test detection of preoperative and postoperative olfactory function, com- pare the preoperative and postoperative 4 weeks, 12 weeks and 24 weeks of olfactory function recovery. Results 10 cases of preoperative were mild impairment of olfactory function, moderate loss of 14 cases, 4 cases of severe loss, 12 cases were loss of the sense of smell, postoperative 4 weeks, 12 weeks and 24 weeks postoperatively, olfactory function gradually improved, especially at 24 weeks, the sense of smell is normal 25 cases, 6 cases of mild impairment, 4 cases were moderate decline, severe impairment in 1 case, 4 cases of loss of the sense of smell, all olfactory function after surgery for patients with a preoperative changed significantly （P〈0.05） . The olfactory T＆T test showed that in 4 weeks after the operation, 30.0% of the patients had a sense of smell improvement. After 12 weeks of surgery, 60.0% of the patients had olfactory improvement, and in the 24 weeks after surgery, 72.5% of the patients had a better sense of smell. Conclusion The functional endoscopic sinus can be improved after surgery, which is of great significance to clinical treatment, which deserves attention and promotion.
