彩色多普勒超声检测头晕患者椎动脉血流量评价疗效分析

Analysis of the efficacy of color doppler ultrasonography on the evaluation of vertebral arterial blood flow in dizziness patients

目的探讨头晕与椎动脉内径及血流量的相关性、探讨头晕患者治疗前后彩色多普勒超声检测椎动脉血流量的改变情况及临床意义。方法应用二维及彩色多普勒超声对比分析头晕患者(观察组)及健康人群(对照组)椎动脉管径及血流量情况,对于头晕患者,使用扩张血管、改善心脑循环血流量及改善微循环等药物治疗10 d后,对比分析治疗前与治疗后椎动脉管径、平均流速及血流量变化情况。结果观察组椎动脉管径及血流量均低于对照组,差异显著(P<0.05),有统计学意义。头晕患者经药物治疗后,椎动脉管径(D)、血流量(Q)及双侧椎动脉血流量之和(S-BFV)较治疗前均有所改善,S-BFV改善率更为显著(P<0.05),差异有统计学意义。结论彩色多普勒超声对头晕患者椎动脉血流量的检测有一定的临床诊断价值,同时也是评价头晕患者治疗效果比较有效的一种直观方法,可作为客观诊断指标和疗效评价指标,值得推广。

Objective To investigate the correlation between dizziness and vertebral artery diameter and blood flow, and to explore the changes and clinical significance of color Doppler ultrasound in detecting vertebral artery blood flow before and after treatment in patients with dizziness. Methods Two dimensional and color Doppler ultrasound contrast analysis of dizzy patients （observation group） and healthy subjects （control group） vertebral artery diameter and blood flow, for the treatment of patients with dizziness, dilation of blood vessels, improve blood supply and blood stasis based drug use, comparative analysis of treatment before and after treatment of vertebral artery diameter, aver- age velocity and the blood flow. Results The vertebral artery diameter and blood flow in the observation group were lower than those in the control group, the difference was significant （P〈0.05） , with statistical significance. Patients with dizziness after drug treatment, vertebral artery diameter （D） , blood flow （Q） and bilateral vertebral artery blood flow （S-BFV） and compared with before treatment were improved, the improvement rate of S-BFV is more significant （P〈0.05） , the difference was statistically significant. Conclusion Color Doppler ultrasound detection of vertebral artery blood flow in patients with dizziness have certain clinical value in the diagnosis, but also evaluate the effect of treatment in patients with dizziness is more effective in an intuitive method, can be used as objective indexes for diagnosis and curative effect evaluation, worthy of promotion.
