摘要
目的探讨血清E-Cad、PGⅠ、CEA和CA19-9单独和联合检测对胃癌诊断的临床价值。方法选取62例胃癌患者(实验组)和65例良性胃病患者(对照组)作为研究对象,比较两组患者血清中E-Cad、PGⅠ、CEA和CA19-9的含量差异,并通过ROC曲线结合Logistic回归评价各指标单独和联合检测对胃癌的诊断价值。结果与对照组比较,实验组血清中E-Cad、PGⅠ含量显著下降,而CEA、CA19-9含量显著升高(P<0.05)。E-Cad、PGⅠ、CEA和CA19-9单独和联合诊断胃癌的ROC曲线下面积分别为0.657,0.621,0.811,0.693和0.939,联合检测曲线下面积显著高于单独检测(P<0.05),最大Youden指数为0.371,0.242,0.435,0.371和0.790,对应的灵敏度为0.613,0.597,0.645,0.597和0.855,特异性为0.758,0.645,0.790,0.774和0.935。结论 E-Cad、PGⅠ、CEA和CA19-9单独检测具有一定的胃癌诊断价值,而联合检测具有较高的临床胃癌诊断价值。
Objective To explore the diagnostic value of serum E-Cad,PG I ,CEA and CA19-9 individual and combined detection of gastric cancer. Methods 62 patients with gastric cancer ( Research group) and 65 patients with benign gastric disorders (Control group) were chosen as research objective. The levels of E-Cad, PG I , CEA and CA19-9 in serum were compared between the 2 groups. And the diagnostic value of individual and combined detection of gastric cancer was evaluated by ROC curve combined with logistic regression. Results Compared with the control group, the serum levels of E-Cad and PG I were significantly decreased, while the CEA and CA19-9 in the research group were significantly increased (P 〈 0.05 ). The area under the ROC curve of E-Cad ,PG I ,CEA and CA19-9 individual and combined detection of gastric cancer were 0.657,0. 621, 0. 811,0. 693 and 0. 939, respectively, and the area of combined detection was higher than that of individual detection (P 〈 0.05). The maximum of Youden index were 0. 371,0. 242,0. 435,0. 371 and 0. 790, corresponding sensitivity were 0. 613, 0. 597 ,0. 645 ,0. 597 and 0. 855, and specificity were O. 758,0.645,0. 790,0. 774 and 0.935, respectively. Conclusion The individual detection of E-Cad,PG I ,CEA and CA19-9 has a certain diagnostic value for gastric cancer,and combined detection had a higher value of clinical diagnosis for gastric cancer.
