小剂量罗哌卡因复合芬太尼用于老年下肢骨折手术麻醉的效果

Effect of low dose ropivacaine combined with fentanyl on anesthesia in elderly patients with lower limb fracture

目的:对小剂量罗哌卡因复合芬太尼用于老年下肢骨折手术麻醉的效果进行探讨与分析。方法:就我院2015年9月~2016年11月收治的100例接受老年下肢骨折手术患者,按照随机、双盲的方式分为观察组、对照组,各50例。对照组接受罗哌卡因,观察组接受小剂量罗哌卡因复合芬太尼,然后分别观察两组麻醉效果以及不良反应的发生情况。结果:两组麻醉效果以及肌松效果比较,观察组肌松优良率为86.00%,显著高于对照组的肌松优良率68.00%,差异显著(P<0.05);除此之外,对两组低血压发生率比较,观察组低血压发生率为70.00%,显著低于对照组的低血压发生率82.00%(P<0.05)。结论:在接受老年下肢骨折手术的患者中,使用小剂量罗哌卡因复合芬太尼,麻醉效果更佳且肌松好、感觉阻滞维持时间长,临床疗效更佳,值得临床推广。

Objective： To investigate and analyze the effect of low dose ropivacaine combined with fentanyl on anesthesia in elderly patients with lower limb fracture. Methods： in our hospital in September 2015 November -2016 period, 100 cases of elderly patients with lower limb bone surgery patients, including random and double blind way, divided into observation group and control group, 50 cases each. The control group received ropivacaine, the observation group received small doses of ropivacainc combined with fentanyl, then observed the anesthetic effect and adverse reactions of the two groups. Results： by comparison, the comparison of two groups of patients with anesthesia and muscle relaxation effect on the patients in the observation group the Matsuyu Ryo muscle rate was 86%, significantly higher than the control group of patients with muscle relaxant excellent rate of 68%, significant difference （P〈0.05）; in addition, occurred in the low blood pressure of two groups of patients were by comparison, patients in the observation group the incidence of hypotension was 70%, significantly lower than the control group the incidence of hypotension 82% （P〈0.05）. Conclusion： the results show that at the time of surgery in elderly patients with lower extremity fractures, using small doses of ropivacaine combined with fentanyl in patients, the anesthetic effect and more muscle relaxation, sensory block maintenance time is long, the clinical curative effect is better accepted by patients, so the value of clinical application and popularization.
