经皮椎间孔镜椎间盘切除术对单阶段腰椎间盘突出患者ODI、MacNab优良率、Lehmann腰椎功能的影响

Effect of percutaneous endoscopic lumbar discectomy of ODI,good rate of Mac Nab and Lehmann lumbar function in patients with single-stage lumbar disc herniation

目的观察经皮椎间孔镜椎间盘切除术对单阶段腰椎间盘突出患者的治疗效果以及对患者Oswestry功能障碍指数(ODI)、MacNab优良率、Lehmann腰椎功能的影响。方法选取2015年1月~2016年4月中国石油天然气集团公司中心医院收治的102例单阶段腰椎间盘突出患者,采用随机数字表法分为观察组和对照组,每组各51例。对照组给予标准后路推板开窗腰椎间盘切除术;观察组给予经皮椎间孔镜椎间盘切除术。比较术后两组ODI、MacNab优良率、Lehmann腰椎功能。结果术后,观察组ODI显著低于对照组,MacNab优良率和Lehmann腰椎功能优于对照组,差异有统计学意义(P<0.05)。结论临床上施行经皮椎间孔镜椎间盘切除术治疗单阶段腰椎间盘突出症,能使术中出血量减少,手术切口减小,住院时间缩短,手术效果提高,同时使患者在术后生活和工作质量得到改善,因此,该技术值得在临床上作进一步推广。

Objective To observe the clinical effect of percutaneous lumbar intervertebral disc resection in the treatment of patients with single-stage lumbar disc herniation and the clinical outcome of Oswestry disability index（ODI）,good rate of Mac Nab and Lehmann lumbar function. Methods From January 2015 to April 2016, 102 patients with single-stage lumbar disc herniation in China National Petroleum Corporation Central Hospital were selected and divided into observation group and control group by random number table, with 51 patients in each group. The control group was treated with standard posterior push plate lumbar disc resection, while the observation group was treated with percutaneous intervertebral disc resection. The ODI, good rate of Mac Nab, Lehmann lumbar function between two groups after operation were compared. Results After operation, ODI in the observation group was lower than that in the control group, and the good rate of Mac Nab and Lehmann lumbar function in the observation group were better than those in the control group, with statistically significant difference（P〈0.05）. Conclusion The treatment of single-stage lumbar disc herniation by percutaneous intervertebral disc resection can reduce the amount of bleeding in operation, decrease the incision of the operation, shorten the hospitalization time, and improve the curative effect and improve quality of life after the operation. Therefore, it is worthy of further clinical promotion.
