关节镜清理术治疗膝关节痛风性关节炎的短期效果

The short-term effect of arthroscopic debridement in treating gout arthritis of knee joint

目的探讨关节镜清理术在膝关节痛风性关节炎治疗中的应用价值。方法回顾性分析2012年1月~2016年12月南方医科大学附属小榄医院51例膝关节痛风性关节炎患者临床资料。其中观察组(19例)应用内科干预、药物+膝关节镜清理术治疗;对照组(32例)给予内科干预、药物治疗。对比两组治疗前后的视觉模拟评分法(VAS)评分和Lysholm关节功能评分,评估患者治疗后6个月的复发率。结果治疗后两组VAS和Lysholm关节功能评分较治疗前均有所改善(P<0.05)。治疗1周后,观察组VAS评分优于对照组(P<0.05)。治疗2周后,观察组VAS评分及Lysholm关节功能评分均优于对照组(P<0.05)。治疗后6个月观察组复发率低于对照组(P<0.05)。结论关节镜清理术用于治疗膝关节痛风性关节炎,短期内能有效缓解疼痛、改善关节功能,降低复发率,可作为内科干预、药物治疗的有效补充手段。

Objective To investigate the value of arthroscopic debridement in the treatment of gouty arthritis of knee joint. Methods The clinical data of 51 patients with gouty arthritis of knee joint treated in Southern Medical University Xiaolan Affiliated Hospital from January 2012 to December 2016 were analyzed retrospectively. The observation group（19 cases） was treated by medical intervention, drug treatment and arthroscopic debridement, while the control group（32 cases） were treated by medical intervention and drug treatment. The visual analogue score（VAS） score and Lysholm joint function score were compared between the two groups before and after treatment, and the rate of recurrence was estimated at 6 months after treatment. Results After treatment, the VAS and Lysholm scores in the two groups were improved（P〈0.05）. One week after treatment, the VAS score in the observation group was better than that in the control group（P〈0.05）. Two week after treatment, the VAS and Lysholm scores in the observation group were better than those in the control group（P〈0.05）. And the recurrence rate of observation group was lower than that in the control group at 6 months after treatment（P〈0.05）. Conclusion Arthroscopic debridement in treating gouty arthritis of knee joint can relieve pain, improve joint function, low recurrence rate, which can be used as effective means of medical intervention and drug treatment.
