从《黄帝内经》探讨益气活血法治疗脱疽所忽视的问题

Neglected Problem of Discussing Qi-tonifying and Blood-activating Method on Treating Sloughing Flat-abscess from Yellow Emperor's Inner Classic

"脱疽"的中医记载最早见于《灵枢·痈疽》,本病病机复杂,缠绵难愈,现代许多医家多用益气活血法治疗,然因病、因人的差异性,不可能一法统之,临床辨证中有几个易被忽视的问题,老者内服辨证尤其要重视脾胃,益气活血辨证时更要兼顾补肾温阳,并配合分期外治处理,内外结合,多方权衡,方显中医"辨证论治"的根本。

Sloughing flat-abscess,originated from Lingshu·Yongju,always lingers on for its complicated pathogenesis. To manage it,healers these years advocated treatment of tonifying Qi and activating blood; however,there is no generalized but customized applications to treat slouging flat-abscess given diverse individuals and diseases. It is noted that,for instance,differentiation on aged population for oral administration suggests emphasis on spleen and stomach,while differentiaiton followed by tonifying Qi and activating blood focuses on comprehensive regime with tonifying the kidney and warming Yang,as well as stagetailored external treatment. In conclusion,it is vital for internal-external combination and integral balance to achieve Chinese syndrome differentiation and treatment.
