半夏白术天麻汤加减辅助治疗急性缺血性脑卒中38例临床疗效观察

Clinical Curative Observation of Using Modified Banxia Baizhu Tianma Decoction in the Treatment of 38 Cases of Acute Ischemic Stroke

目的:观察分析半夏白术天麻汤加减辅助治疗急性缺血性脑卒中38例临床疗效。方法对我院在2014年2月~2015年4月期间收治的19例急性缺血性脑卒中患者纳入研究对象设为对照组并给予依达拉奉治疗,同前将我院在2015年6月~2016年8月期间收治的19例急性缺血性脑卒中患者纳入研究对象设为观察组并给予在对照组治疗的基础上联合半夏白术麻汤加减辅助治疗,治疗两周后对照分析两组患者治疗效果、神经功能恢复及日常生活活动情况、中医证候计分及相关血液流变学指标变化。结果:两组患者治疗前ADL评分及CBF、NIHSS评分及中医症候积分比较均无明显差异(P>0.05);治疗后两组患者ADL评分及CBF、NIHSS评分及中医症候积分均明显优于治疗前(P<0.05),且观察组患者治疗后ADL评分及CBF、NIHSS评分及中医症候积分改善程度明显优于对照组(P<0.05),观察组患者治疗后临床总有效率(94.63%)明显高于对照组(36.84%)(P<0.05)。结论:半夏白术天麻汤加减辅助治疗急性缺血性脑卒中临床疗效较显著,有效地改善患者日常生活能力及局部脑血流量,从而促进神经功能的恢复,对患者的活动能力提高具有重要意义,值得临床推广。

Objective： To observe the clinical efficacy of Banxia Baizhu Tianma Decoction in the treatment of 38 patients with acute ischemic stroke. Methods： 19 patients with acute ischemic stroke admitted to our hospital from February 2014 to April2015 were selected as subjects and were set as controlled group and treated with edaravone. At the same time,19 cases of acute ischemic stroke patients were selected as subjects admitted to our hospital from June 2015 to August 2016 and were set as observation group and given Banxia Baizhu Decoction combined anesthesia adjuvant therapy based on the treatment of the controlled group.After two weeks of treatment,the treatment effect,neurological function recovery and daily living activity,TCM syndrome score and related hemorheology index were compared and analyzed. Results： No significant differences were detected in the treatment effect,neurological function recovery and daily living activity,TCM syndrome score,ADL score,CBH,NIHSS score and TCM syndrome score between the two groups after two weeks of treatment. After treatment,ADL score and CBF,NIHSS score and TCM syndrome score were significantly better than those before treatment（P〈0. 05）. The positive rate of ADL score,CBF,NIHSS score and TCM syndrome score were significantly better than those in the controlled group（P〈0. 05）. The total effective rate（94. 63%） in the observation group was significantly higher than the controlled group（36. 84%）（P〈0. 05）. Conclusions： The clinical effect of Banxia Baizhi Tianma Decoction in the treatment of acute ischemic stroke is significant and effective. The daily life ability and local cerebral blood flow can be effectively promoted,so as to promote the recovery of neurological function. It is important to improve the ability of patients to improve their activities,which is worthy of clinical promotion.
