平衡舒筋手法结合中医护理对脑卒中康复期患者ADL能力影响

Effect of Balanced Tendon-relaxing Manipulation Combined with Traditional Chinese Medicine Nursing on ADL for Patients with Subacute Stroke

目的:观察平衡舒筋结合中医护理对脑中风康复期患者上肢运动功能和生活能力的影响。方法:80例亚急性期脑卒中偏瘫患者分为治疗组(40例)和对照组(40例),在常规药物及康复护理干预基础上,治疗组接受中医平衡舒筋手法推拿治疗,共持续8周。结束后通过Fugl-Meyer量表上肢部分(FMA-UE)评定上肢运动功能;Barthel指数(MBI)评估患者日常生活能力(ADL);Wolf运动功能评定(WMFT)评定患者上肢及手的精细运动功能。结果:治疗结束后2组患者FMA-UE评分上肢、腕、手得分显著改善(P<0.05),对照组总分(51.70±3.42),治疗组(56.98±4.52),改善效果明显优于对照组(P<0.05)。2组MBI和WMFT评分较治疗前均有显著改善(P<0.05),对照组患者治疗后MBI和WMFT评分分别为(73.12±9.92)和(43.95±4.43),治疗组(80.56±10.15)和(49.92±5.93),改善效果均显著优于对照组(P<0.05)。结论:中医推拿结合综合护理比明显促进脑卒中亚急性期患者上肢运动功能改善,提高日常生活能力。

Objective： To observe the effect of Chinese massage combined with dialectical nursing on recovery of activities of daily living（ADL） in patients with subacute stroke. Methods： 80 patients with subacute stroke hemiplegia were divided into treatment group（n=40） and controlled group（n= 40）,basis on conventional drugs and comprehensive rehabilitation,the treatment group received Pingheng Shujin Method of Chinese massage. After 8 weeks treatment,Fugl-Meyer scale upper limb part（FMA-UE）,Modify Barthel index（MBI） and Wolf motor function evaluation were measured to compared the therapeutic effect. Results： After treatment,the score of upper limb,wrist and hand of FMA-UE significantly improved,the total score of the controlled group was（51. 70±3. 42）,while the treatment group was（56. 98±4. 52）,which was significantly difference（P〈0. 05）. MBI and WMFT scores of were significantly improved（P〈0. 05）. The scores of MBI and WMFT were（73. 12±9. 92） and（43. 95±4. 43） in the controlled group and（80. 56±10. 15） and（49. 92±5. 93） in the treatment group respectively,the improvement effect was significantly better than the controlled group（P〈0. 05）. Conclusions： Chinese massage combined with comprehensive rehabilitation can significantly improve the motor function of upper limb and daily life ability in patients with subacute stroke.
