复方苦参针治疗肝内胆汁淤积性肝病的临床研究

Clinical study of compound kushen injection in the treatment of intrahepatic cholestasis of live

目的:观察复方苦参针治疗肝内胆汁淤积性肝病的临床疗效。方法:将120例肝内胆汁淤积性肝病患者随机分成2组各60例,对照组给予口服熊去氧胆酸胶囊,用量:10 mg/kg·d-1,分2次口服,静脉注射S-腺苷蛋氨酸,用量:1.0 g/d,1次/d;治疗组在对照组用药的基础上加用复方苦参注射液,用量:20 ml/d,加入氯化钠注射液200 ml静脉滴注。两组均4周为1疗程。疗程为4周,观察治疗前后临床疗效、谷氨酰转肽酶(GGT)、血清碱性磷酸酶(ALP)、肝功能[谷丙转氨酶(ALT)、谷草转氨酶(AST)、血清白蛋白(ALB)、总胆红素(TBIL)]、血清肝纤维化[透明质酸(HA)、Ⅲ型前胶原(PC-Ⅲ)、Ⅳ型胶原(Ⅳ-C)、层粘蛋白(LN)]、凝血功能[凝血酶原时间(PT)、凝血酶原活动度(PTA)]等指标变化。结果:与治疗前相比较,治疗后二组病例临床疗效、GGT、ALP、肝功能、肝纤维化均明显改善(P<0.05),其中治疗组除AST外(P>0.05),余指标改善程度均明显优于对照组(P<0.05)。结论:复方苦参针对肝内胆汁淤积性肝病具有良好的治疗效果。

Objective: To observe the clinical efficacy of compound kushen injection in the treatment of intrahepatic cholestasis of liver. Methods: 120 patients with intrahepatic cholestasis type liver disease were randomly divided into 2 groups,60 cases in each group. The control group was given conventional treatment,and the treatment group was treated with compound kushen injection on the basis of the treatment of the control group. The course of treatment was four weeks. Before and after treatment,to observe the change of the clinical curative effect、glutamyl peptidase( GGT)( GGT),serum alkaline phosphatase( ALP),liver function( glutamic-pyruvic transaminase( ALT) and glutamic-oxalacetic? transaminase( AST),serum albumin( ALB),total bilirubin( TBIL)) and serum liver fibrosis indexes( hyaluronic acid( HA),procollagen III( PC-III),type IV collagen( IV-C) and laminin( LN)). Results: Compared with before treatment,after treatment,two cases of clinical curative effect,GGT,ALP,liver functional indice and hepatic fibrosis indice were significantly improved( P < 0. 05). In the treatment group,except AST( P >0. 05),the improvement of other indexes was obviously better than that of the control group( P < 0. 05). Conclusion: compound kushen injection is effective in the treatment of intrahepatic cholestasis of liver.
