流程化管理在经股动脉插管全脑血管DSA患者围术期护理中的应用

Application of process management in perioperatively nursing patients undergoing femoral artery intubation and cerebral digital subtraction angiography

目的探讨流程化管理在经股动脉插管全脑血管数字减影血管造影(DSA)患者围术期护理中的应用。方法行全脑血管DSA和支架置入治疗的脑血管疾病患者240例分为两组。2016年的120例(试验组)采用流程化管理,接受经过流程化处理的DSA检查前后的护理干预。其内容包括成立DSA流程化管理小组、制定护理管理流程和针对DSA检查相关并发症建立应急预案。2015年的120例(对照组)采用常规术前准备及术后护理。比较两组术后3d局部和全身并发症发生率,出院时调查患者对DSA手术护理满意度。结果试验组患者术后局部和全身并发症发生率均低于对照组(P<0.05和P<0.01)。试验组患者对DSA手术的满意度高于对照组(97.50%vs.91.67%)(P<0.05)。结论在经股动脉插管全脑血管DSA患者实施围术期流程化管理护理能显著提高护理质量,降低术后并发症发生率。

Objective To study the application of process management in perioperatively nursing the patients undergoing femoral artery intubation and cerebrovascular digital subtraction angiography （DSA）. Methods Cerebrovascular DSA and stent implantation via femoral artery intubation were performed in 240 patients with cerebrovascular diseases, who were equally divided into two groups of A （operated in the year of 2015） and B （operated in the year of 2016）. The process management included seting up DSA process management team, developing nursing management process and establishing emergency plan for the complications related to DSA examination. The incidences of local and systemic complications in 3 days after operation were compared between two groups. The satisfaction of patients with DSA operation was investigated at discharge, Results The incidences of local and systemic complications in 3 days after operation were lower in group B than those in group A（P〈0. 05 and P〈0. 01）. The satisfaction rate of patients with DSA operation was higher in group B than that in group A（97.50% vs. 91.67%）（P〈0. 05）. Conclusion The application of process management in perioperatively nursing the patients undergoing femoral artery intubation and eerebrovascular DSA can effectively improve nursing quality and reduce the incidence of DSA-related complications.
