耐碳青霉烯类肠杆菌的耐药机制及基因分型分析 被引量:2

摘要 目的分析耐碳青霉烯类肠杆菌(CRE)的耐药机制及基因分型。方法 7491株肠杆菌采用PCR检测CRE碳青霉烯酶基因,并对扩增产物进行测序,分析各自构成比,全自动细菌分析仪鉴定菌株,纸片扩散法测试CRE药敏率。结果 7491株肠杆菌中检测到CRE共52株,检出率为0.69%;其中以大肠埃希菌和肺炎克雷伯菌构成比最高,分别为42.31%与32.69%。CRE对阿米卡星和头孢吡肟的耐药率最低,分别为5.77%和23.08%。52株CRE中全部检出碳青霉烯酶基因,其中KPC型、IMP型、GES型、OXA型阳性率分别为55.77%、40.38%、3.85%、11.54%。结论CRE构成比最高的是大肠埃希菌和肺炎克雷伯菌,耐药基因主要为KPC和IMP基因型,临床上需在药敏试验的指导下有目的地用药。
