单孔腹腔镜下经肛门微创手术治疗直肠肿瘤的疗效分析（附21例报告）

Curative Effect Analysis of Transanal Minimally Invasive Surgery Under Single-port Laparoscopy for Rectal Tumor（a Report of 21 Cases）

为探讨单孔腹腔镜经肛门微创手术（TAMIS）治疗直肠肿瘤的安全性及有效性，2015年6月至2017年3月，对我院21例直肠肿瘤患者行单孔腹腔镜下TAMIS治疗。选用适当的体位，行肠壁全层或黏膜下切除，保证切缘完整，创面连续缝合关闭。分析单孔腹腔镜下TAMIS在直肠肿瘤治疗中的应用效果。结果显示，21例患者直肠病变均获完整切除，平均手术时间为（50．6±30．5）min，术后平均住院时间为（2．5±1．6）d。术后患者出现便血1例，绒毛状管状腺瘤复发1例，无严重并发症发生。结果表明，单孔腹腔镜下TAMIS治疗直肠肿瘤具有经济、安全、有效的优点。

In order to explore the validity and safety of transanal minimally invasive surgery （TAMIS）for rectal tumor,21 patients with rectal tumor in our hospital received TAMIS under single-port laparoscopy from June 2015 to March 2017.The patients were positioned properly,the tumor was removed by endoscopic whole-layer resection or the submucosal resection.The resection margins of tumors were ensured integration.The wound surface was closed by a running suture.The application effect of TAMIS under single-port laparoscopy for rectal tumor were analyzed. As results, the rectal lesion of all 21 patients was removed completely.The mean operative time was （50.6±30.5） min. The mean postoperative hospital stay was （2.5 ±1.6） days.One case was found hematochezia after operation and one case villous tubular adenoma recurrence. No other severe complications were found. Results show that TAMIS is inexpensive, safe, and effective for the treatment of rectal tumor.
