早期乳腺癌患者行保乳术和改良根治术的近远期治疗效果分析

Analysis on Short-Term and Long-Term Curative Effects of Early-stage Breast Cancer Patients Given Breast-Conserving Surgery and Modified Radical Operation

目的比较早期乳腺癌患者行保乳术和改良根治术的近远期治疗效果。方法回顾性分析2009年1月至2011年12月我院收治的74例早期乳腺癌患者临床资料,并按接受手术方式不同分为保乳术组与改良根治术组,比较两组手术切口长度、手术时间、术中出血量及术后住院时间等围手术期指标,并比较两组术后1个月、6个月生活质量评分及5年局部复发率、远处转移率、生存率。结果保乳术组较改良根治术组切口长度明显缩短,手术时间、术中出血量及术后住院时间均明显减少(P<0.05);保乳术组较改良根治术组术后1个月及术后6个月生活质量评分均明显提高(P<0.05);两组局部复发率、远处转移率及5年生存率比较无显著差异(P>0.05)。结论在早期乳腺癌治疗中,保乳术具有创伤小、术后恢复快等优势,有效提高患者生活质量,且远期疗效与改良根治术相当。

Objective To compare the short-term and long-term curative effects of breast-conserving surgery and modified radical operation in treating early-stage breast cancer patients.Methods 74 cases of early-stage breast cancer patients admitted and treated in our hospital during January 2009 and November 2011 were selected as the study objectives. Their clinical data were retrospectively analyzed.They were divided into breast-conserving surgery group and modified radical operation group according to the patients ＇ different ways of operations.Compare the incision length,operation time,a mount of bleeding during operation,post-operative hospital stay length and other indexes in the peri operative period.Compare the life quality scores of one month and six months after operation as well as the regional relapse rate,distant metastasis rate and survival rate of within 5 years.Results The incision length in the breast-conserving surgery group was significantly shorter than the modified radical operation group.And the operation time,a mount of bleeding during operation and post-operative hospital stay length in the breast-conserving surgery group were all significantly less than the modified radical operation group（P〈0.05）.The life quality scores of one month after operation and six months after operation in the breast-conserving surgery group were significantly more increased than those in the modified radical operation group（P〈0.05）.The comparison of the regional relapse rate,distant metastasis rate and survival rate of within 5 years between both groups showed no significant differences（P〉0.05）.Conclusion In the early-stage breast cancer treatment methods,breast-conserving surgery can cause less wound and promote the post-operative recovery.It can effectively improve the patients ＇ life quality.The breast-conserving surgery have similar long-term curative effects with modified radical operation.
