半夏泻心汤治疗寒热错杂慢性胃炎合并消化性溃疡随机平行对照研究

Banxia Xiexin Decoction in the Treatment of Mixed Cold and Chronic Gastritis Peptic Ulcer Complicated with Randomized Controlled Study

[目的]观察半夏泻心汤治疗寒热错杂慢性胃炎合并消化性溃疡疗效。[方法]使用随机平行对照方法,将90例患者按就诊顺序抽签随机分为两组。对照组45例培垛普利,奥美拉唑20mg/次,2次/d;阿莫西林1mg/次,2次/d;甲硝唑0.4mg/次,2次/d。治疗组45例半夏泻心汤:半夏、旋履花各9g,干姜、黄连各3g,炒党参12g,炒枳壳6g。虚寒明显者,加附子;呕吐严重加重生姜用量;湿热、结聚突出,加大黄;气滞病症,去黄连及半夏。连续治疗2周为1疗程。观测临床症状、疼痛程度(VAS)评分、HAMD评分、HAMA评分、不良反应。治疗1疗程(2周),判定疗效。[结果]治疗组痊愈20例,显效12例,有效10例,无效3例,总有效率93.33%;对照组痊愈15例,显效9例,有效11例,无效10例,总有效率77.78%;治疗组疗效优于对照组(P<0.05)。VAS评分、HAMD评分、HAMA评分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]半夏泻心汤治疗寒热错杂慢性胃炎合并消化性溃疡,疗效满意,无严重不良反应,值得推广。

[Objective] To observe the curative effect of Banxia Xiexin decoction in treating mixed cold and chronic gastritis peptic ulcer. [Methods] Using random parallel control method, 90 patients were randomly divided into two groups according to the order of visit. The control group 45 cases, perindopril, omeprazole20 mg/times, 2 times/d, amoxicillin 1 mg/times, 2 times/d, metronidazole 0.4 mg/times, 2 times/d. The treatment group of 45 cases of Banxia Xiexin decoction： Banxia, Xuanfuhua each 9 g, Ganjiang, Huanglian each 3 g, Chaodangshen 12 g, Chaozhiqiao 6 g. Xuhan worse, add Fuzi; Vomiting serious increase the dosage by Shengjiang; Shire, Jieju worse, add Dahuang; Qizhi, subtract Huanglian, Banxia. Continuous treatment for 2 weeks is the 1 course of treatment. The clinical symptoms, degree of pain（VAS） score, HAMD score, HAMA score and adverse reaction were observed. The course of treatment was 1 weeks（2 weeks）, and the curative effect was determined. [Results] the treatment group 20 cases were cured, 12 cases markedly effective, 10 cases effective, 3 cases ineffective, the total efficiency of 93.33%; control group 15 cases were cured, 9 cases markedly effective, 11 cases effective, 10 cases ineffective, the total efficiency of 77.78%; the treatment group than the control group（P0.05）. The VAS score, HAMD score and HAMA score were improved in the two groups（P0.01）, and the treatment group was better than that of the control group（P0.01）. [Conclusion]The effect of Banxia Xiexin decoction in treating mixed cold and chronic gastritis peptic ulcer, with no serious adverse reactions, worthy of promotion.
