原发性高血压患者自主神经功能与血清一氧化氮的相关性 被引量:11

The relationship between autonomic nervous function and serum nitric oxide in patients with essential hypertension

摘要 目的:探讨原发性高血压患者自主神经功能与血清一氧化氮(NO)的相关性。方法:连续纳入原发性高血压患者284例,以血清NO二分位数进行分组(NO-1组≤38.04μmol/L,NO-2组>38.04μmol/L),对所有患者行血清NO测定及24h动态心电图监测,收集血清NO值和心率变异性(HRV)的频域分析指标VLF、LF、HF、LF/HF及时域分析指标SDNN、SDANN、rMSSD、pNN50,分析血清NO与HRV之间的关系。结果:Pearson相关分析显示,血清NO与年龄、24hSBP、24hDBP、LF、LF/HF分别呈负相关(r=-0.248、-0.488、-0.368、-0.506、-0.454,均P<0.05),与HF、SDANN呈正相关(r=0.263、0.454,均P<0.05)。多元线性回归分析显示,24hSBP、HF、LF为血清NO的独立影响因素。结论:原发性高血压患者血清NO降低与收缩压升高、交感神经过度激活和副交感神经抑制有关。 Objective:To investigate the relationship between autonomic nervous function and serum nitric oxide(NO)in patients with essential hypertension.Method:A total of 284 subjects were enrolled and divided into two groups according to serum NO concentration(NO-1≤38.04μmol/L,NO-238.04μmol/L).The value of serum NO and heart rate variability(HRV)frequency domain indexes VLF,LF,HF,LF/HF and time domain indexes SDNN,SDANN,rMSSD,pNN50 were collected.Then the association between serum NO and HRV was explored.Result:Pearson's correlation analysis showed that age,24 hSBP,24 hDBP and LF,LF/HF were negatively correlated with serum NO(r=-0.248,-0.488,-0.368,-0.506,-0.454 respectively,all P0.05),while HF,SDANN were positively correlated with serum NO(r=0.263,0.454,both P0.05).Multiple linear regression revealed that 24 hSBP,LF and HF were independent influential factors of serum NO.Conclusion:The decrease of serm NO is associated with elevated systolic blood pressure,sympathetic overdrive and parasympathetic inhibition in patients with essential hypertension.
