自动痔疮套扎术联合外剥内扎术治疗混合痔的临床研究

Ruiyun Procedure for Hemorrhoids Combined with External Dissection ＆ Internal Ligation in the Treatment of Mixed Hemorrhoids

为探讨自动痔疮套扎术联合外剥内扎术治疗混合痔的临床疗效,将146例混合痔患者按随机数字表法随机分为治疗组和对照组,各73例,治疗组采用自动痔疮套扎术加外剥内扎术治疗,对照组采用PPH加外剥内扎术治疗.对比2组治愈率、手术时间、术中出血量、住院时间、术后并发症,以及手术前后肛管测压情况.结果显示,2组治愈率、住院时间比较差异均无统计学意义,P〉0.05;但治疗组在手术时间、术中出血量、术后并发症发生率方面均明显优于对照组,P〈0.05.2组手术前后肛管最大收缩压、直肠感觉容量、直肠最大容量比较差异均无统计学意义,P〉0.05;但2组术后肛管静息压均明显降低,治疗组降低更明显,P〈0.05,更接近正常值.结果表明,自动痔疮套扎术联合外剥内扎术治疗混合痔疗效确切,与PPH联合外剥内扎术相比,具有手术时间短、术中出血少、术后并发症少等优点,值得临床推广应用.

This study was to explore the clinical effect of Ruiyun procedure for hemorrhoids combined with external dissection internal ligation in the treatment of mixed hemorrhoids,randomly divided 146 cases of mixed hemorrhoids into treatment group and control one,73 cases for each,which the former received above-mentioned procedure,meanwhile the latter received PPH plus external dissection internal ligation for mixed hemorrhoids;then compared both group＇s following indexes such as curative rate, operative time, intraoperative bleeding volume, hospitalization time, postoperative complication, and anal pressure before and after operation.As results,in curative rate and hospitalization time there was no statistical difference between the two groups（ P〈0.05）,but in operative time,intraoperative bleeding volume and incidence of postoperative complication the former was all superior to the latter（ P 〈0.05）;in the two groups there was no statistical difference between pre-and postoperative measured values of anal max. systolic pressure, of rectal sensory capacity,of rectal max. capacity（ P 〉0.05）, bUt after operation both group＇s anal rest pressure was all significantly decreased,and the former＇s decrease was more significant（ P 〈0.05）, more closed to normal value.Results show that the combination procedure used in the former for mixed hemorrhoids has certain effect,and shorter operative time,less intraoperative bleeding volume,less complication than the ones used in the latter.It is worth to be popularized and used.
