慢性心力衰竭患者检测血清基质金属蛋白酶组织抑制物、可溶性E-选择素的意义

Significance of detection of serum tissue inhibitor of metalloproteinase-4 and soluble E-selectin in patients with chronic heart failure

目的:探讨联合检测血清基质金属蛋白酶组织抑制物(TIMP-4)、可溶性E-选择素(SES)、血清游离脂肪酸(FFA)水平在预测慢性心力衰竭(CHF)患者终点事件中的应用价值。方法:选取120例CHF患者作为研究组,选取同期健康体检者120例作为对照组,使用DAS-ELISA法测定2组血清TIMP-4、SES水平,使用免疫比浊法测定血清FFA水平。结果:研究组血清TIMP-4、SES、FFA水平明显高于对照组(均P<0.05)。Pearson直线相关分析显示:TIMP-4与SES、FFA呈正相关(r=0.512、0.522,均P<0.001),SES与FFA呈正相关(r=0.536,P<0.001)。ROC曲线分析显示:两项检测的AUC、灵敏度与特异度均明显高于单项检测,而三项检测的AUC、灵敏度与特异度均明显高于两项检测(均P<0.05)。结论:联合检测血清TIMP-4、SES、FFA水平在预测慢性心力衰竭患者终点事件中的应用价值显著。

Objective：To study the application in the prediction of outcome events by combined detection of serum tissue inhibitor of metalloproteinase-4（TIMP-4）,soluble E-selectin（SES） and free fatty acid（FFA） levels in patients with chronic heart failure（CHF）. Methods：A total of 120 cases of CHF served as study group,and at the same time 120 cases of healthy controls as control group. Serum TIMP-4 and SES levels in two groups were detected by DAS-ELISA,and serum FFA level by immune turbidimetric determination. Results：Serum TIMP-4,SES and FFA levels in study group were significantly higher than those in control group（all P〈0.05）. Pearson straight correlation analysis showed TIMP-4 was positively correlated with SES and FFA（r = 0. 512,0. 522; all P 0. 001）,and SES was positively correlated with FFA（r = 0. 536,all P 0. 001）. ROC curve analysis showed AUC,sensitivity and specificity of combined detection of two indicators were significantly higher than those of single detection,but AUC,sensitivity and specificity of combined detections of three indicators were significantly higher than those of combined detection of two indicators（all P〈0.05）. Conclusion：The applied value in the prediction of outcome events by combined detection of serum TIMP-4,SES and FFA levels in patients with CHF is significant.
