血清25-(OH)D3水平与2型糖尿病周围神经病变的关系 被引量:3

摘要 目的:分析血清25-(OH)D3水平与2型糖尿病周围神经病变(DPN)的相关性。方法:选择高州市石鼓镇中心卫生院2014年7月至2016年10月收治的145例2型糖尿病患者为研究对象,并根据是否合并周围神经病变分为DPN组87例、单纯糖尿病(NDPN)组58例,比较两组患者一般资料,包括性别、年龄、病程、脂糖代谢生化指标及血清25-(OH)D3水平,比较两组间各指标差异,分析25-(OH)D3水平与上述指标及周围神经病变的相关性。结果:DPN组与NDPN组血清25-(OH)D3水平依次是(36.14±8.59)nmol·L^(-1)、(57.48±12.63)nmol·L^(-1),NDPN组含量明显高于DPN组,差异具有统计学意义(P<0.05);比较两组一般资料、生化指标等,显示病程、糖化血红蛋白(Hb Alc)、低密度脂蛋白-胆固醇(LDL-C)等各项指标比较,差异具有统计学意义(P<0.05);血清25-(OH)D3与上述差异性指标进行Pearson相关性分析,显示病程、Hb Alc及LDL-C水平与血清25-(OH)D3呈负相关;血清25-(OH)D3(<50 nmol·L^(-1))是周围神经病变独立危险因子。结论:糖尿病合并周围神经病变者血清25-(OH)D3明显降低,呈负相关,其中血清25-(OH)D3与患者病程、Hb Alc及LDL-C水平呈负相关,可能与疾病发生、发展有关。 Objective To analyze the relationship between serum 25-hydroxy vitamin D level and type-2 diabetic peripheral neuropathy.Methods:A total of 145 patients with type-2 diabetes in our hospital were taken as the clinical research objects,and they were divided into type-2 diabetic peripheral neuropathy group(DPN group,87 cases)and non diabetic peripheral neuropathy group(NDPN group,58 cases)based on whether the merger of peripheral neuropathy.The general information(including gender,age,course of disease),biochemical indicators of lipid metabolism and serum 25-hydroxy vitamin D[25-(OH)D3]level in the two groups were compared,and the correlation between 25-(OH)D3 level,the above indexes and peripheral neuropathy were analyzed.Results:The serum 25-(OH)D3 level in the DPN group was(36.14±8.59)nmol/L which was remarkably lower than that in the NDPN group(57.48±12.63)noml/L(P0.05);there were signifi cant differences in general information,biochemical indicators,course of disease,Hb Alc and LDL-C between the two groups(P0.05);the correlation analysis between serum 25-(OH)D3 and the above indexes performed by Pearson analysis showed that the course of disease,Hb Alc and LDL-C levels were negatively correlated with serum 25-(OH)D3,and serum 25-(OH)D3(50nmol/L)was an independent risk factor for peripheral neuropathy.Conclusion:Serum25-(OH)D3 which may be related to the occurrence and development of the disease,is signifi cantly decreased in diabetic patients with peripheral neuropathy,and negatively correlated with the course of disease,Hb Alc and LDL-C.
