经阴道子宫输卵管四维超声造影评价输卵管通畅性的价值 被引量:18

摘要 目的探讨经阴道子宫输卵管四维超声造影技术(TVS 4D-Hycosy)评价实时、动态检测输卵管通畅度的临床应用价值。方法选取2015年6月至2016年10月在黄石市中心医院诊断的不孕症女性患者46例,均行TVS 4D-Hycosy和子宫输卵管X线碘油造影(HSG)检查。比较两种方法的检查结果、不良反应发生率。结果 46例患者共检查输卵管87条,单侧输卵管切除5条。TVS 4D-Hycosy显示:输卵管通畅31条(35.63%),输卵管通而不畅33条(37.93%),输卵管阻塞23条(26.44%);HSG显示:输卵管通畅32条(36.78%),输卵管通而不畅30条(34.48%),输卵管阻塞25条(28.74%)。两种检查方法总符合度为94.25%(82/87),差异无统计学意义(P>0.05)。四维超声造影检查和X线碘油造影检查不良反应发生率分别为15.22%、17.39%,两者比较差异无统计学意义(P>0.05)。结论 TVS 4DHycosy检查输卵管阻塞安全、有效,可作为临床上评估不孕症患者输卵管通畅性的有效手段。 Objective To evaluate the efficiency of transvaginal four-dimensional hysterosalpingo-contrast sonography(TVS 4D-Hycosy) in real-time,dynamic testing of tubal patency.Methods A total of 46 infertile women,who underwent TVS 4D-Hycosy detection of uterus and fallopian tubes as well as X-ray angiography(hysterosalpingography,HSG) check of uterus oviduct at Department of Ultrasonography of the Central Hospital of Huangshi City from June 2015 to October 2016,were selected as the research subjects.The detection results and negative reactions of the two methods were compared.Results A total of 87 fallopian tubes were detected in the 46 infertile women,and unilateral fallopian tube removal was detected in 5 cases.TVS 4D-Hycosy detection results showed the patency of fallopian tubal was good in 31 cases(35.63%),poor in 33 cases(37.93%),and obstruction was detected in 23 cases(26.44%).HSG detection results showed the patency of fallopian tubal was good in 32 cases(36.78%),poor in 30 cases(34.48%),and obstruction was detected in 25 cases(28.74%).Results conformity was 94.25%(82/87) for the two detection methods(P0.05).Adverse reactions occurred in 15.22% and 17.39% patients undergoing TVS 4D-Hycosy and HSG detection,showing no significant difference between the two methods(P0.05).Conclusion TVS 4D-Hycosy is a safe and effective detection for the evaluation of fallopian tubal patency in infertile women,and it is worthy of application in clinic work.
