COPD急性加重期血浆sTREM-1与肺功能及凝血功能的关系

The relationship among sTREM-1,lung function and coagulation status in the blood plasma of patients with acuteexacerbation chronic obstructive pulmonary disease

目的观察慢性阻塞性肺疾病急性加重期(acute exacerbation chronic obstructive pulmonary disease,AECOPD)患者血浆可溶性髓样细胞触发受体-1(soluble triggering receptor expressed on myeloid cells 1,sTREM-1)与肺功能及凝血功能之间的关系。方法将2016年6月-2017年6月新疆医科大学第一附属医院呼吸科收治的105例AECOPD患者作为试验组,同期30名健康体检者为对照组。收集一般人口学资料、动脉血气及肺功能的相关指标、血清sTREM-1、凝血酶时间(PT)、部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、血清D-二聚体(D-D)、血小板(PLT)。对两组患者的sTREM-1、肺功能及凝血指标进行对比分析。结果 AECOPD组的sTREM-1、PT、APTT、FIB、D-D明显高于健康对照组,并且差异有统计学意义(P<0.05),但PLT未见明显差异;sTREM-1与PT、FIB呈明显正相关(P<0.05);随着肺功能的加重,sTREM-1增高。结论血浆sTREM-1具有评估AECOPD患者肺功能及凝血功能状态的潜在价值。

Objective To explore the relationship among soluble triggering receptor expressed on myeloid cells 1(sTREM-1)and lung function,coagulation status in the blood plasma of patients with acute exacerbation chronic obstructive pulmonary disease(AECOPD).Methods A total of 105 AECOPD patients were admitted into the First Affiliated Hospitalof Xinjiang medical university from June 2016 to June 2017.And another 30 healthy volunteers were selected as control group.The data of demographics,arterial blood gas,sTREM-1,PT,APTT,FIB,D-D,P1 atelet count and pulmonary function parameters were collected and analyzed.Spearman rank correlation was used for correlation analysis for sTREM-1,lung function,coagulation count of both groups.Results The level of sTREM-1,TF,PT,APTT,FIB,D-D in plasma of AECOPD patients were significantly higher than the control group(P <0.05),but the PLT has no difference between two groups.Plasma sTREM-1 was significantly positively correlated with PT and FIB.sTREM-1 was increased with the increase of pulmonary function.Conclusion Plasma sTREM-1 in patients with AECOPD is a promising biomarker to evaluate pulmonary function and coagulation status in AECOPD patients.
