推拿联合外用药治疗肝郁气滞型乳腺增生症的疗效及对患者E_2、P、PRL、LH的影响

Curative effect of massage combined with external medication in the treatment of hyperplasia of mammary glands and the influence E_2,P,PRL and LH

目的分析推拿联合外用药治疗肝郁气滞型乳腺增生症的疗效及对患者E_2、P、PRL、LH的影响,探讨其临床适用性。方法选择2016年5月-2017年5月于新疆维吾尔自治区中医医院就诊的136例肝郁气滞型乳腺增生症患者,按简单随机住院号的方法分为治疗组(68例)和观察组(68例),观察组患者给予散结乳癖膏外敷治疗,治疗组患者在观察组基础上联合推拿疗法。治疗2个月观察2组患者的疗效改善情况。采用视觉模拟疼痛评价(VAS评分)评价2组患者治疗前后疼痛症状改善情况。同时观察2组患者治疗前后性激素水平主要包括黄体生成素(LH),雌二醇(E_2),催乳素(PRL)及孕激素(P)的改善情况及随访患者的复发情况。结果治疗组患者治疗后的总有效率为97.06%,观察组患者的总治疗有效率为82.35%,差异有统计学意义(χ~2=3.981 3,P=0.046 0)。2组患者治疗前VAS评分比较,差异无统计学意义(P>0.05),治疗后2组VAS评分均显著降低(P<0.05),且治疗组患者治疗后VAS评分显著低于观察组患者(P<0.05)。2组患者治疗前LH,E_2,PRL及P水平均无明显差异(P>0.05),治疗后均明显改善(P<0.05),且治疗组患者各项激素水平均优于观察组患者(P<0.05)。治疗组患者治疗后5个月后复发率为41.18%(28/68),观察组组患者为17.65%(12/68),2组患者的复发率比较差异有统计学意义(χ~2=4.533 3,P=0.033 2)。结论推拿联合外用药治疗肝郁气滞型乳腺增生症的临床效果显著,可以有效改善患者的性激素水平,对改善乳腺增生症的预后较好。

Objective To analyze the clinical effect of massage combined with external medication in the treatment of hyperplasia of mammary glands and the influence on E_2,P,PRL and LH.Methods 136 inpatients with hyperplasia of mammary glands from May 2016 to May 2017 in our hospital were randomly divided into two groups according to the hospitalized number and each group included 68 cases.The observation group were treated with external medication,while the treatment group patients combined with massage therapy.Two groups of patients were treated and observed for two months.The curative effect of the two groups was observed.The VAS score,E2,P,PRL and LH before and after treatment in the two groups were observed.Results The total effective rate in observation group after treatment was 97.06%,while the total effective rate in the control group was 82.35%,and the difference was statistically significant(χ~2=3.981 3,P =0.046 0).The VAS score in two groups of patients before treatment were not statistically different(P >0.05).After the treatment,VAS score was significantly decreased(P <0.05)in both group,and VAS score of the patients in the treatment group was lower than the observation group,and the difference was statistically significant(P <0.05).Before the treatment,E2,P,PRL and LH of two groups of patients were not statistically significant(P >0.05).After treatment,E_2,P,PRL and LH were significantly improved(P<0.05),and those indexes of the patients in the treatment group were better than the observation group,and the difference was statistically significant(P <0.05).After treatment,the recurrence rate in treatment group 41.18%,of the recurrence rate of the observation group was17.65%,and the difference was statistically significant(χ~2=4.533 3,P =0.033 2).Conclusions The clinical effect of massage combined with external use of drugs in the treatment of breast hyperplasia is remarkable,which can effectively improve the level of sex hormone in patients and has a good prognosis of improving hyperplasia of mammary glands.
