临床路径在混合痔手术治疗过程中的应用效果

Effect of clinical pathway in the treatment of Hemorrhoids

目的探讨将临床路径(clinical pathway,CP)应用于混合痔治疗的临床效果。方法选取2017年1月至2017年6月广西壮族自治区民族医院肛肠外科收治的180例混合痔患者为研究对象,随机分成CP组(n=90,采用临床路径管理模式)和NCP组(n=90,采用传统管理模式)。比较两组术前准备时间、术后住院时间及总住院时间,各项费用及药比。结果 CP组术前准备时间、术后住院时间、总住院时间均显著少于NCP组,差异均有统计学意义(均P<0.05)。CP组在药品费用、检验检查费、总住院费用、抗菌药物费用均显著少于NCP组,差异均有统计学意义(均P<0.05);两组手术费用及材料费用差异均无统计学意义(均P>0.05)。CP组药显著低于NCP组,差异有统计学意义(P<0.05)。结论将临床路径应用于混合痔的治疗过程中,能有效减少患者的住院费用、缩短住院时间及控制药比,在规范医疗行为的同时也利于更为充分高效地利用医疗资源。

Objective To study the effect of clinical pathway（CP） in the treatment of mixed hemorrhoids. Methods 180 patients with mixed hemorrhoids who were treated at Department of Anus Intestine surgery of the Guangxi Zhuang Autonomous Region National Hospital from January 2017 to June 2017 were recruited. They were randomly assigned to CP group（n = 90, clinical pathway management mode） and NCP group（n = 90, using traditional management mode）. The pre-operation preparation time, post-operation hospitalization duration and total hospitalization duration, total costs and the drug costs ratio were compared between the two groups.Results The pre-operation preparation time, post-operation hospitalization duration and total hospitalization duration in CP group were significantly less than those in NCP group（all P〈 0.05）. In CP group, total drug costs, investigation costs, total hospitalization expenses and antibiotics costs were significantly less than those in NCP group（all P〈 0.05）. There was no statistically significant difference in operation costs and operation material costs between the two groups（all P 〉0.05）. Total drug costs in the CP group was significantly lower than in the NCP group（P〈0.05）. Conclusion Clinical pathway treatment mode can effectively reduce hospitalization expenses, shorten hospitalization time and control drug cost ratio in the treatment of mixed hemorrhoids. It facilitates standardized medical behaviors and at the same time enables effective use of medical resources.
