摘要
目的:探究降糖方联合西药对妊娠糖尿病妇女脂肪因子的影响,旨在为临床治疗妊娠糖尿病提供新思路。方法:126例妊娠糖尿病患者,按照随机数法分成对照组和实验组。对照组患者在常规护理的基础上增加西药治疗,主要包括精蛋白生物合成人胰岛素注射液及二甲双胍片联合治疗。实验组患者在对照组治疗的基础上增加降糖方进行治疗,观察比较两组患者的临床资料和血脂生化指标、血糖控制情况、脂肪因子、不良反应等数据。结果:实验组患者与对照组患者在身高、体质量、BMI、TG、TC等数据比较均无统计学意义(P〉0.05)。治疗前对照组与实验组的血糖值分别为(12.92±2.34)mmol/L、(13.21±2.13)mmol/L,两组空腹血糖值对比不具有统计学意义;治疗后对照组与实验组的血糖值分别(9.65±0.55)mmol/L、(7.23±0.42)mmol/L,对照组患者的血糖值高于实验组(P〈0.05)。两组患者在治疗前的RBP-d、APN、TNF-α的数据比较差异不具有统计学意义(P〉0.05),治疗后实验组患者的RBP-d、TNF-α数据均小于对照组(P〈0.05);治疗后实验组患者的APN数据大于对照组(P〈0.05)。实验组患者的不良反应的总发生率为36.51%低于对照组患者不良反应的总发生率47.62%(P〈0.05)。结论:降糖方联合西药能够降低妊娠糖尿病妇女的血糖、RBP-4及TNF-α水平,提高患者的APN水平,对脂肪因子的影响较为显著。
Objective: To study the effect of Jiangtang Decoction on adipokines in women with gestational diabetes mellitusand provide new ideas for clinical treatment of gestational diabetes mellitus. Methods: 126 cases of gestational diabetes according to the random number method were divided into control group and experimental group. The patients in the control group on the basis of conventional care added the treatment of Western medicine,mainly including protamine biosynthetic human insulin injection and metformin tablets combined therapy. The patients in the experimental group were treated with Jiangtang Decoction on the basis of the control group. The clinical data and blood lipid biochemical indexes,blood glucose control,fat factor and adverse reaction were observed and compared between the two groups. Results: There were no significant differences in height,body mass,BMI,TG or TC between the experimental group and the control group( P 0. 05). The blood glucose levels of the control group and the experimental group were( 12. 92 ± 2. 34) mmol/L and( 13. 21 ± 2. 13) mmol/L respectively. There was no significant difference between the two groups. The blood glucose level was( 9. 65 ± 0. 55) mmol/L and( 7. 23 ± 0. 42) mmol/L,respectively. The blood glucose level of the control group was higher than that of the experimental group( P 0. 05). The levels of RBP-d,APN and TNF-α in the two groups were not statistically significant( P 0. 05). The levels of RBP-d and TNF-α in the experimental group were lower than those in the control group( P 0. 05). The APN levels of the experimental group were significantly higher than those of the control group( P 0. 05). The overall incidence of adverse events was 36. 51% in the experimental group and 47. 62%( P 0. 05) in the adverse reactions of the control group. Conclusion: Jiangtang Decoction combinedwith western medicine can decrease the levels of blood glucose,RBP-4 and TNF-α in pregnant women with diabetes mellitus and improve the level of APN and the effect on fat factor.
