摘要
目的 探讨急性坏死性胰腺炎 (acute necrotizing pancreatitis,ANP)时肾微循环的动态变化及其与肾损害的关系。方法 SD大鼠 12 8只 ,随机分为对照组与 SAP组 ,用 5 %牛磺胆酸钠胰腺被膜下均匀注射制成 ANP模型。采用放射性生物微球技术在制模后 1/ 2 h、2 h、6 h、12 h分别测定肾血流量 ,同时观察肾静脉血 TXA2 / PHI2 比值及肾功能和肾组织学改变。结果 与对照组相比 ,ANP组各时相肾血流量均明显降低 ,BU N、Cr明显增高 ,TXA2 / PGI2 2小时起明显增高。 ANP组内肾血流量比较 ,2 h时较 1/ 2 h有所回升 ,6 h又复下降 ,12 h降至最低。对照组肾组织检查无明显异常 ,ANP组 1/ 2 h即有组织学损害 ,并随时间延长而更加严重。结论 肾损害在 ANP早期即已发生 ,肾脏缺血与缺血再灌注损伤引起的肾脏微循环障碍可能为
Objective To explore the relationship between the changes of renal damage and microcirculation in rats with acutes necrotizing pancreatitis(ANP).Methods 128 rats were divided randomily into control and ANP group.The model of ANP was established by injection of 5% sodium taurocholate solution into pancreatic capsule,Radioactive biomicrosphere technique was used to measure the blood flow of kidney at 1/2h,2h,6h,12h after induction of ANP,Meanwhile,the samples from renal vein and tissues were collected to observe the following changes of renal function.TXA 2/PGI 2 and renal histology.Results Contrast with the control group,the kidney blood flow in ANP group was deckrased significantly and plasma levels of BUN?Cr were increased significantly at 1/2h,2h,6h and 12 h after the induction of ANP,The ratio of TXA 2/PGI 2 of ANP group were significantly higher than that in control group from 2h after ANP.In the ANP group,the blood flow of kidney was higher at 2h than at 1/2h,then was lower again at 6h and was lowest at 12h.Renal pathological changes were observed at 1/2h after ANP,and was aggravated constantly with the course of disease.Conclusion Renal damag are present in ANP early,the renal microcirculation disorder caused by renal ischemia and ischemina reperfusion injury may be one of the comsiderable reasons of renal damage in ANP
