睡眠呼吸暂停低通气综合征与重度COPD患者睡眠监测SO_2临床分析

摘要 目的比较32~69岁睡眠呼吸暂停低通气综合征(SAHS)与重度慢性阻塞性肺疾病(COPD)患者睡眠监测血氧饱和度(SO_2)异同,分析两种疾病夜间缺氧的特点。方法收集SAHS和重度COPD患者各30例为病例组,同时收集30例与之匹配的正常人作为对照组,均采用美国伟康Stardust睡眠监测系统进行整夜的睡眠监测。结果 SAHS组与对照组的夜间基础SO_2、夜间最低SO_2、夜间最长氧降时间均有显著性差异,P<0.05。重度COPD组与对照组的夜间基础SO_2、夜间最低SO_2、夜间最长氧降时间均有显著性差异,P<0.05。SAHS组与重度COPD组的年龄、BMI、夜间基础SO_2均无显著性差异,P>0.05。夜间最低SO_2、夜间最长氧降时间均有显著性差异,P<0.05。SAHS组、重度COPD组夜间最长氧降时间在SAHS组与重度COPD组中呈正相关,P<0.05。结论 SAHS患者与重度COPD患者均有夜间缺氧,但两者又有不同,SAHS患者较重度COPD患者夜间间歇缺氧程度更明显,而SAHS患者缺氧时间较重度COPD患者短。 Objective To compare the similarities and differences of sleep monitoring the blood oxygen saturation(SO2) between sleep apnea hypoventilation syndrome(SAHS) and severe chronic obstructive pulmonary disease(COPD) among patients from 32 to 69 years old, and analyze the characteristics of two kinds of disease of oxygen at night. Methods Patients with SAHS or severe COPD were collected, with 30 cases for each group. At the same time, 30 cases of healthy person were collected as the control group. All cases were monitored sleep all night through American wei kang Stardust sleep monitoring system. Results Based SO2, night minimum SO2 and the longest oxygen reduction time between SAHS group and control group all had significant difference( P 〈 0.05). And they all had significant difference between severe COPD group and control group too( P 〈 0.05). Age, BMI, nightly basis of SO2 between SAHS and severe COPD group were no significant difference(P 〉 0.05), but minimum SO2 and the longest oxygen reduction time at night had significant difference(P 〈 0.05). The longest nightly oxygen dereasing time in SAHS group and severe COPD group were positively correlated(P〈 0.05).Conclusions SAHS patients and severe COPD patients both have anoxia at night, but they are different. SAHS patients is more obvious than severe COPD patients, and SAHS patients had shorter anoxia time than severe COPD patients.
