摘要
目的探讨间歇导尿和膀胱功能训练辅助穴位针灸对脊髓损伤神经源性膀胱的效果。方法将我院2015年2月至2016年5月收治的98例脊髓损伤神经源性膀胱患者随机分为对照组和辅助组,每组49例。对照组患者给予穴位针灸治疗,辅助组给予间歇导尿和膀胱功能训练辅助穴位针灸治疗。观察两组患者治疗前、后的最大排尿量、残余尿量情况及尿动力学结果及泌尿系统感染和肾积水发生情况。结果治疗后,两组患者治疗后的最大排尿量升高,残余尿量降低,且辅助组上述指标均优于对照组(P<0.05)。两组患者治疗后的VH2O、Pves、Pdet、BC水平均显著改善,且辅助组患者的上述指标优于对照组(P<0.05)。辅助组患者的泌尿系统感染和肾积水发生率均低于对照组(P<0.05)。结论间歇导尿和膀胱功能训练辅助穴位针灸治疗脊髓损伤神经源性膀胱可有效恢复患者膀胱功能,改善尿动力学结果,降低并发症发生率。
Objective To investigate the effect of intermittent catheterization and bladder function training assisted by point acupuncture in spinal cord injury neurogenic bladder. Methods From February 2015 to May 2016, 98 patients with spinal cord injury neurogenic bladder in our hospital were divided into control group and adjuvant group, with 49 cases in each group. Patients in the control group were treated with point acupuncture, while those in the adjuvant group were given intermittent catheterization and bladder function training assisted by point acupuncture. The maximum urinary volume,residual urine volume and urodynamic results before and after treatment, incidences of urinary tract infections and hydronephrosis were compared in the two groups. Results After treatment, the maximum urinary volume increased and residual urine volume decreased in both groups, and those of the adjuvant group were better than the control group(P<0.05). The levels of VH2 O, Pves, Pdet and BC after treatment in both groups significantly improved, and the above indexes in the adjuvant group were better than the control group(P<0.05). The incidence of urinary tract infection and hydronephrosis in the adjuvant group were lower than those in the control group(P <0.05). Conclusion Intermittent catheterization and bladder function training assisted by point acupuncture in spinal cord injury neurogenic bladder can effectively restore bladder function of patients, and improve urodynamic results, reduce the incidence of complications.
