单纯25G玻璃体切割和25G玻璃体切割联合内界膜剥除及移植治疗黄斑裂孔的疗效对比观察

Observation of effects of single 25G vitrectomy and 25G vitrectomy combined with internal limiting membrane peeling and in treatment of macular hole

目的探讨单纯25G玻璃体切割和25G玻璃体切割联合内界膜剥除及移植治疗黄斑裂孔的疗效,为单纯25G玻璃体切割和25G玻璃体切割联合内界膜剥除及移植对治疗黄斑裂孔的疗效对比提供理论依据。方法选取2013年1月—2015年8月在医院接受诊治的86例黄斑裂孔患者作为此次研究对象,随机平均分成观察组和对照组,各43例,对照组患者采用单纯25G玻璃体切割进行治疗,观察组患者在对照组的基础上联合内界膜剥除及移植进行治疗,分析两组患者手术后视力恢复水平情况,分析两组患者手术后黄斑裂孔闭合情况,分析两组患者术后并发症情况。结果观察组总有效率为95.35%,对照组为74.42%,观察组明显要好,两组数据资料存在较大差异,具有统计学意义(P<0.05);观察组总闭合率为88.37%,对照组为65.11%,观察组明显要好,两组之间的数据资料存在较大差异,具有统计学意义(P<0.05);观察组并发症的总发生率为9.29%,对照组为25.58%,观察组明显要低于对照组,两组之间的数据资料存在较大差异,具有统计学意义(P<0.05)。结论采用25G玻璃体切割联合内界膜剥除及移植治疗黄斑裂孔患者疗效显著,患者的视力恢复能力强,黄斑裂孔闭合率非常好,术后并发症相对较少,在临床治疗黄斑裂孔患者时值得应用推广。

Objective To compare the effect of single 25G vitrectomy and 25G vitrectomy combined with internal limiting membrane peeling effect and transplantation in the treatment of macular hole,provide theoretical basis for the treatment.Methods From January 2013 to August 2015,the hospital for treatment of 86 cases of macular hole patients as the study were randomly divided into observation group and control group,43 patients in the control group were treated with pure 25G vitrectomy performed treatment,the observation group were combined with the control group on the basis of the internal limiting membrane peeling and transplantation therapy,analyzed visual acuity level of patients after surgery groups,analyzed two groups of patients after surgery macular hole closure,analyzed two groups of patients after surgery complications.Results The total effective rate was 95.35% in observation group,the control group was 74.42%,significantly better than the comtrol group,the difference was statistically significant（P 0.05）; observation group was 88.37% of total closure,the control group was 65.11%,significantly better than the control group,difference was statistically significant（P 0.05）; Observation group total complication rate was 9.29%,the control group was 25.58% observation group was significantly lower than the control group,the difference was statistical significance（P 0.05）.Conclusion 25G vitrectomy combined with internal limiting membrane peeling and transplantation in the treatment of patients with macular hole has good efficacy,the closure rate is low,with few postoperative complications,worthy of application and promotion.
