循环式腹腔内热灌注化疗对进展期胃癌的临床疗效研究

Clinical Efficacy Research of Cyclic Intraperitoneal Hyperthermic Perfusion Chemotherapy for Progressive Gastric Cancer

目的评价循环式腹腔内热灌注化疗对进展期胃癌的治疗效果。方法选择2013年1月-2015年1月在瓦房店市中心医院进行手术治疗的进展期胃癌患者96例作为研究对象，随机分为观察组和对照组各48例。观察组患者进行术后循环式腹腔热灌注化疗联合全身化疗，对照组仅进行单纯全身化疗。随访观察并比较两组患者的复发率、生存率及不良反应发生情况。结果两组1年复发率及生存率差异无统计学意义（P〉0．05）；观察组的3年复发率低于对照组，3年生存率高于对照组，差异有统计学意义（P〈0．05）。两组患者胃肠道反应、血小板减少、粒细胞减少、切口感染及口腔炎等不良反应的发生率差异无统计学意义（P〉0．05）。结论循环式腹腔内热灌注化疗可改善进展期胃癌肿瘤患者术后的长期预后，未增加不良反应的发生。

Objective To investigate the clinical efficacy of intraperitoneal hyperthermic perfusion chemotherapy for progressive gastric cancer. Methods A total of 96 progressive gastric cancer patients undergoing operation in Wafangdian Center Hospital were collected from January, 2013 to January, 2015. All patients were randomly divided into two groups： study group （n=48） and control group （n=48）. The patients in study group received intraperitoneal hyperthermic perfusion chemotherapy combined with systemic chemotherapy, while the patients in control group only received simple systemic chemotherapy. The recurrence rate, survival rate, and adverse reaction rate between two groups were compared under follow-up. Results There were no significant differences in one-year recurrence rate and survival rate between two groups （P〉0.05）. The three-year recurrence rate in study group was lower than control group, and the three-year survive rate in study group was higher than control group （all P〈0.05）. The adverse reaction rate of gastrointestinal reaction, thrombocytopenia granulocytopenia, incision infection and stomatitis were no significant different between two groups （P〉0.05）.
