重组人脑钠肽联合托拉塞米治疗急性心肌梗死并心力衰竭的临床疗效

Clinical effect of recombinant human brain natriuretic peptide combined with torasemide in the treatment of acute myocardial infarction with heart failure

目的探讨重组人脑钠肽联合托拉塞米治疗急性心肌梗死并心力衰竭的临床疗效。方法选取福建医科大学附属宁德市医院2013年10月—2016年9月收治的急性心肌梗死并心力衰竭患者58例,随机分为对照组(n=29)和观察组(n=29)。对照组患者给予硝普钠治疗,观察组患者在对照组基础上给予重组人脑钠肽联合托拉塞米治疗。比较两组患者的临床疗效及治疗前后心率、血压、血浆B型脑钠肽(BNP)变化情况,观察患者不良反应发生情况。结果观察组患者临床疗效优于对照组(P<0.05)。治疗前,两组患者心率、舒张压、收缩压及BNP水平比较,差异无统计学意义(P>0.05);治疗后,观察组患者心率、舒张压、收缩压及BNP水平低于对照组(P<0.05)。观察组患者不良反应发生率低于对照组(P<0.05)。结论采用重组人脑钠肽联合托拉塞米治疗急性心肌梗死并心力衰竭的临床疗效确切,可有效改善患者的心功能,降低血压,且安全性高。

Objective To evaluate the clinical effect of recombinant human brain natriuretic peptide combined with torasemide in the treatment of acute myocardial infarction with heart failure.Methods A total of 58 cases of acute myocardial infarction with heart failure patients were selected from October 2013 to September 2016 in Ningde City Hospital Affiliated to Fujian Medical University,which were randomly divided into control group（n = 29） and observation group（n = 29）.Control group was treated with sodium nitroprusside,observation group was treated with recombinant human brain natriuretic peptide combined with torasemide.The clinical effect and heart rate,blood pressure,BNP before and after treatment were compared between the two groups,and the incidence of adverse reactions were observed.Results The clinical effect of observation group was better than control group（P〈0.05）.Before treatment,no significant differences of heart rate,diastolic blood pressure,systemic blood pressure or BNP was found between the two groups（P〈0.05）; after treatment,the observation group of heart rate,diastolic blood pressure,systemic blood pressure and BNP were lower than control group（P〈0.05）.The incidence rate of adverse reactions in observation group was lower than control group（P〈0.05）.Conclusion Recombinant human brain natriuretic peptide combined with torasemide have an exactly clinical effect in the treatment of acute myocardial infarction with heart failure,it can effevtively improve the cardiac function,lower blood pressure,and with high safety.
