肩峰切线位片的应用研究

Study the usage of tangential position radiograph in acromion

目的 通过肩峰前缘骨赘发生率及骨赘增生程度与临床表现相关性比较 ,探讨肩峰前后切线位 X线射片的临床应用价值。方法 371个有慢性肩部疼痛史或有肩部阳性体征的肩作为实验组 ,16 9个无肩痛史无肩部阳性体征的肩作为对照组。两组同时行肩峰前后切线位 X线摄片 ,按大小及形态特征将骨赘分为 0°、 °、 °、 °。根据骨赘分度分布情况进行统计 ,两组行秩和检验。两组骨赘发生率、 ° °所占比率及 °所占比率分别行 X2 检验。结果 实验组与对照组秩和检验有高度显著性差异 (P <0 .0 1) ,两组骨赘发生率 X2 检验有高度显著性差异 (P <0 .0 1) , ° °骨赘所点比率 X2 检验有高度显著差异 (P <0 .0 1)。 °骨赘所占比率 X2 检验有高度显著性差异 (P <0 .0 1)。结论 肩峰前缘骨赘发生率及骨赘增生程度与临床表现密切相关。肩峰前后切线位 X线摄片能清楚显示肩峰前缘骨赘 。

Objective To study clinical practical values of anteroposterior tangential position radiograph in acromion through practical values of the correlation between frequencies,degrees of acromion anterior margin ostephytes and clinical manifestations.Methods Three hundreds seventy-one shoulders with chronic pains or with positive signs in physical examinations were put in experimental group and one hundred sixty-nine shoulders without pains and positive signs were in control group.Acromion anteroposterior tangential position radiographs were taken in both groups.Osteophytes in radiographs were divieded into 0°?Ⅰ°?Ⅱ° and Ⅲ° according to their shapes and sizes.The distributions of the degreesd were treated with rank sum test.Frequencies of osteophytes in both groups,ratio of Ⅱ°?Ⅲ° and ratio of Ⅲ° were treated with X 2 tese.Results There was prominent different(P<0.01)in both groups treated with rank sum test,and prominent difference(P<0.01)occurred in frequencies of osteophytes in both groups treated with X 2 test.Prominent difference in frequencies of Ⅱ° ?Ⅲ° osteophytes.And there was prominent difference in frequencies of Ⅲ° osteophytes(P<0.01).Conclusion There is close correlation between the frequencies and degrees of acromion anterior margin osteophytes and clinical manifestation.Acromion anteroposterior tangential position radiograph can reveal acromion anterior margin osteophyte,and it can be used as a routine item for patients who complain pains in shoulders.
