综合医院焦虑抑郁量表在艾滋病病人中的应用与评价

Chinese version of hospital anxiety and depression scale:psychometric properties in AIDS patients

目的在艾滋病（AIDS）病人中,对中文版综合医院焦虑抑郁量表（HADS）的内部一致性和因子结构进行检测,进而分析其用于艾滋病病人的可行性。方法在指定时间内对艾滋病病人进行随机抽样,得到425例有效样本,记录其年龄、性别并填写中文HADS;Cronbach’sα系数用于检验量表的内部一致性;验证性因子分析用于分析中文HADS的因子结构,并用赤池信息准则等6个指标评价7个拟建模型的拟合优度。结果 bifactor（2group-factors）模型对中文HADS拟合优度最高,加入条目表述因子后,各模型的拟合优度均有提高;HADS内部一致性较好,尤其是在bifactor模型中。结论研究结果支持bifactor模型,表明将HADS作为整体量表可较好地用于艾滋病病人。

Objective To evaluate the psychometric properties of the Chinese version of Hospital Anxiety and Depression Scale（HADS）in terms of internal consistency reliability and factor structure in AIDS patients,and further discuss the feasibility of applying HADS in AIDS population.Methods 436 AIDS patients were randomly enrolled in this study,where 425 patients completed the Chinese version of HADS.Cronbach＇sαcoefficient was used to evaluate the internal consistency of HADS.The factor structure was analyzed by confirmatory factor analysis and six indices,such as Akaike information criterion,used for the goodness-of-fit comparison of seven popular models.Results The bifactor（2 group-factors）model showed the best goodness-of-fit among the 7 models.After adding the item wording method factor,goodness-of-fit for the 7 models was all improved.HADS showed a good internal consistency,especially for the bifactor model.Conclusion The bifactor model of HADS is a satisfactory model when being used as a measurement for general distress in AIDS population.
