胺碘酮在冠心病快速心律失常中的应用及对心电变化的影响

摘要 目的探讨胺碘酮治疗冠心病快速心律失常患者的治疗效果及其对心电变化的影响。方法选取本院2011年3月至2014年10月收治的100例冠心病快速心律失常患者作为研究对象,按随机数字表法分为观察组、对照组,每组50例,观察组给予胺碘酮治疗,对照组给予利多卡因治疗,比较两组治疗效果,观察两组不良反应发生率及两组心电变化。结果观察组、对照组治疗后有效率分别为94.0%、78.0%,观察组有效率高于对照组,对比差异具有统计学意义(P<0.05);治疗后两组室性早搏数、短阵室速数均有明显改善,与治疗前差异有统计学意义(P<0.05),治疗后观察组室性早搏数、短阵室速数明显低于对照组、QTc间期明显长于对照组,差异有统计学意义(P<0.05);观察组治疗后不良反应发生率仅为6.0%,显著低于对照组26.0%,对比差异具有统计学意义(P<0.05)。结论采用胺碘酮治疗冠心病快速心律失常,疗效确切,可更好地降低室性早搏及短阵室速数,改善患者心功能,且用药安全性可靠。 Objective To investigate the clinical effect of amiodarone in the treatment of tachyarrhythmia in patients with coronary heart disease and its effect on electrocardiographic changes. Methods A total of 100 patients with coronary heart disease and tachyarrhythmia who were admitted to our hospital from March 2011 to October 2014 were enrolled and divided into observation group and control group using a random number table, with 50 patients in each group. The patients in the observation group were given amiodarone, and those in the control group were given lidocaine. The clinical outcome was compared between the two groups, and the incdience rate of adverse events and electrocardiographic changes were observed in both groups. Results After treatment, the observation group had a significantly higher response rate than the control group(94.0% vs 78.0%, P 0.05). Both groups had significant improvements in the numbers of ventricular premature beats and paroxysmal ventricular tachycardia after treatment(P〈0.05), and comapred with the control group, the observation group had significantly lower numbers of ventricular premature beats and paroxysmal ventricular tachycardia and a significantly longer QTc interval(P〈0.05). The observation group had a significantly lower incidence rate of adverse events than the control group(6.0% vs 26.0%, P 0.05).Conclusion Amiodarone has a marked clinical effect in the treatment of tachyarrhythmia in patients with coronary heart disease and can reduce the numbers of ventricular premature beats and paroxysmal ventricular tachycardia and improve patients' cardiac function. It is also safe and reliable.
