冠状动脉钙化病变特点及其与高血压PPI ABI的相关性

Characteristics of coronary artery calcification and its correlation with PPI and ABI in patients with essential hypertension

目的探讨冠状动脉钙化病变特点及其与高血压动态脉压指数(PPI)及踝臂指数(ABI)的相关性。方法将收治的疑似冠状动脉钙化患者220例,依照是否存在高血压将患者分为观察组及对照组;采用东芝Aquilion one320排容积CT对患者行冠状动脉CTA检查,依照冠状动脉CT扫描检查结果及AHA分类标准对患者检查结果进行评估。结果冠状动脉钙化病变除可见轻度或中度狭窄合并部分/点状钙化,重度狭窄合并血管钙化外,也可见轻度、中度、重度狭窄合并弥漫性/多发血管钙化;高血压组患者ABI评分结果与冠状动脉钙化评分(CACS)评分呈显著负相关关系(P<0.05),ABI评分与CACS无相关性(P>0.05);观察组患者PPI评分与CACS评分正性相关(P<0.05),但对照组患者PPI评分与CACS评分无显著相关关系(P>0.05)。结论冠状动脉钙化常合并血管狭窄,且其冠状动脉钙化程度与高血压患者PPI、ABI呈正性相关。

Objective To analyze the correlation between coronary artery calcification and PPI and ABI in hypertensive patients. Methods 220 patients with suspected coronary artery calcification were enrolled in this study. Patients were divided into observation group and control group according to the presence or absence of hypertension. Coronary arterial CTA was performed on patients with CTO. Results Coronary artery calcification lesions could be seen in mild or moderate stenosis with partial / punctate calcification, visible severe stenosis with vascular calcification, and also seen mild, moderate, severe stenosis with diffuse / multiple vascular calcification. There was a significant negative correlation between the score and the CACS score （P＜0.05）. There was no correlation between the ABI score and the CACS score （P＞0.05）. The PPI score of the observation group was positively correlated with the CACS score （P＜0.05）. There was no significant correlation between PPI score and CACS score （P＞0.05）. Conclusion Coronary calcification often associated with vascular stenosis, and the degree of coronary artery calcification and hypertensive patients with PPI, ABI positive correlation.
