腹腔镜手术治疗胃间质瘤的临床应用

Clinical Application of Laparoscopy Operation in Treating Gastric Stromal Tumor

目的探讨腹腔镜手术在治疗胃间质瘤的临床疗效和安全性。方法回顾2010年12月至2016年4月在我院接受手术治疗的35例胃间质瘤患者。根据手术方式不同,将患者分为开腹组和腔镜组。比较两组患者的术中情况、术后恢复情况和病理学结果。结果两组患者手术时间差异无统计学意义(P=0.340),但开腹组的出血量显著多于腔镜组(P=0.002)。而腔镜组的排气时间、饮食恢复时间和住院时间均显著短于开腹组(P<0.05)。两组患者病理情况比较,肿瘤直径、核分裂象和细胞形态的差异均无统计学意义(P>0.05)。结论腹腔镜手术治疗胃间质瘤能够有效减少手术创伤,提高恢复速度,但对手术安全并没有影响,值得在临床上进一步推广和应用。

Objective To investigate the clinical efficacy and safety of laparoscopy in the treatment of gastric stromal tumors. Methods 35 cases of gastric stromal tumor in our hospital from December 2010 to April 2016 were reviewed. According to different surgical method, the patients were divided into laparotomy group and laparoscopic group. Intraoperative conditions, postoperative re- covery and pathological results were compared between the two groups. Results There was no significant difference in the operation time between the two groups （P=0. 340） , but there was significantly larger amount of bleeding in the laparotomy group than that of the laparoscopic group （P = 0. 002 ）. Exhaust time, diet recovery time and hospitalization time of the laparoscopic group were significantly shorter than those of the laparotomy group （P〈O. 05 ）. There was no significant difference in pathological comparison, tumor diameter, mitotic figure and cell morphology between the two groups （ P〉0. 05 ）. Conclusion Laparoscopic surgery for gastric stromal tumors can effectively reduce surgical trauma and improve recovery, with no side effect on safety, which is worthy of further promotion and clinical application.
