内镜超声检查辅助下套扎术治疗胃肠道小间质瘤的疗效观察和安全性分析

Observation and safety analysis of endoscopic ultrasonography in the treatment of small mesothelioma of gastrointestinal tract

[目的]评估内镜超声检查辅助下套扎术治疗胃肠道小间质瘤的疗效观察和安全性。[方法]回顾性分析胃肠道小间质瘤患者55例,研究组36例经内镜超声检查辅助下套扎术治疗,对照组19例经常规开腹手术治疗,比较2组患者的临床疗效、手术时间、术中出血量、术后开始活动时间、住院时间及手术复发情况,对比2组患者术后消化道出血、疼痛、消化道穿孔等并发症的发生情况。[结果]研究组治疗总有效率(94.4%)高于对照组(89.5%),但差异无统计学意义。2组患者手术时间差异无统计学意义。与对照组比较,研究组术中出血量、术后开始活动时间、住院时间均显著降低,差异有统计学意义(P<0.05)。研究组术后并发症发生率(5.6%)显著低于对照组(31.6%),差异有统计学意义(P<0.05)。研究组术后复发率(0)低于对照组(5.3%),差异无统计学意义。[结论]超声内镜辅助下套扎术对胃肠小间质瘤的临床治疗有良好疗效和安全性。

[Objective]To evaluate the efficacy and safety of endoscopic ultrasonography in the treatment of small mesothelioma of gastrointestinal tract.[Methods]55 patients with gastrointestinal stromal tumors were retrospectively analyzed.36 cases in study group were treated with endoscopic ultrasonography assisted ligation,and 19 cases in control group were treated with conventional laparotomy.Compared the clinical efficacy,operation time,intraoperative blood loss,postoperative activity time,length of stay,surgical recurrence,postoperative gastrointestinal bleeding,pain,gastrointestinal perforation in the two group.[Results]The total effective rate（94.4% ）was higher in the study group than in the control group（89.5% ）,but the difference was not statistically significant.There was no significant difference in the operation time between the two groups.Compared with the control group,the intraoperative blood loss,the time of operation and the length of hospital stay were significantly lower in the study group（P0.05）.The incidence of postoperative complications（5.6% ）in the study group was significantly lower than that in the control group（31.6% ）,the difference was statistically significant（P0.05）.The recurrence rate（0% ）was lower in the study group than in the control group（5.3% ）,the difference was not statistically significant.[Conclusion]Endoscopic ultrasonography has a good curative effect and safety in the clinical treatment of gastrointestinal stromal tumors.
