低剂量多层螺旋CT最大密度投影在尘肺病中的应用 被引量:6

摘要 目的探讨低剂量多层螺旋CT最大密度投影在尘肺病中的应用。方法采取随机抽样法选取60例于2016年1月~2016年12月间在我院接受CT检查的尘肺患者作为研究对象,对所有患者分别进行常规剂量多层螺旋CT检查和低剂量多层螺旋CT检查(分别简称为常规剂量组和低剂量组),并对所采集的常规层厚的图像进行最大密度投影重建。对两组方法下的图像质量进行评价,对图像的p、q、s和t小阴影进行评价,对两组不同剂量下患者的辐射情况进行比较。结果两组图像质量存在较为明显的差异,具有统计学意义(P<0.05);以常规剂量下检出的层厚5.0mm的p、q、s、t小阴影为基准,结果显示虽然低剂量组在各项阴影的检出与常规剂量比较差异均具有统计学意义(P<0.05),但低剂量组也能正常显示所有的小阴影,并未出现遗漏现象;低剂量组的各项辐射剂量均明显低于常规剂量组,差异均具有统计学意义(P<0.05)。结论低剂量多层螺旋CT最大密度投影技术在尘肺病的诊断中不仅可以达到筛查图像、鉴别诊断的临床最基本要求,又能够显著减少受检查者的辐射风险,提高了患者的治疗依从性,因此值得在临床上广泛推广应用。 Objective To explore the application of low density multi-slice spiral CT maximum density projection in pneumoconiosis. Methods 60 patients with pneumoconiosis underwent CT examination from January 2016 to December2016 were selected as the study object by random sampling. All patients were treated with conventional multi-slice spiral CT and low-dose multi-slice spiral CT examination( referred to as the conventional dose group and low dose group,respectively),and the collection of conventional thickness of the image of the maximum density projection reconstruction,the image quality of the two groups of methods was evaluated,the p,q,s and t small shadows of the images was evaluated,and the radiation of the two groups of patients under different doses were compared. Results There were significant differences in the image quality between the two groups,the difference as significant( P〈0. 05). Based on the p,q,s and t small shades with the thickness of 5. 0 mm detected at the conventional dose,the results showed that although the difference between the shadow detection of the conventional dose group and the low dose group was statistically significant( P〈0. 05),but the low dose group could show all the small shadows normally,and there was no omission. The dose of radiation in the low dose group was significantly lower than that in the conventional dose group,and the difference was statistically significant( P〈0. 05). Conclusion Low-dose multi-slice spiral CT maximum density projection technique can not only achieve the most basic clinical requirements of screening images and differential diagnosis,but also can significantly reduce the radiation risk of the subjects and improve the patient's compliance,it is worth to widely used in clinical practice.
