不同分娩方式对高龄初产妇与经产妇新生儿窒息的影响

Effect of different delivery methods on neonatal asphyxia in advanced age primipara and multipara

目的探讨不同分娩方式对高龄初产妇与经产妇新生儿窒息的影响。方法选取我院收治的高龄初产妇87例作为观察组,以同期收治的高龄经产妇98例作为对照组。比较两组产妇经阴道生产与剖宫产对新生儿窒息发生率的影响。结果对照组阴道分娩率为45.92%,略高于观察组的43.68%;对照组新生儿窒息率为6.12%,略低于观察组的6.90%,组间差异不显著(P>0.05)。两组新生儿Apgar评分差异不显著(P>0.05)。观察组阴道分娩新生儿的窒息率(5.26%)略低于剖宫产的窒息率(8.16%),组内差异不显著(P>0.05)。结论高龄初产妇可根据产妇的基本状态选取最适合的分娩方式,剖宫产并非其唯一选择,适当增加高龄初产妇的阴道分娩率,并结合严密的监护措施,能够降低新生儿的不良事件。

Objective To investigate the different delivery methods on neonatal asphyxia in advanced age primipara and multipara. Methods A total of 87 cases of advanced age primipara in our hospital were enrolled as the observation group,and 98 cases of advanced age multipara at the same period were enrolled as control group. The effects of vaginal delivery and cesarean section on neonatal asphyxia were compared. Results The rate of vaginal delivery in the control group was45.92%, which was slightly higher than 43.68% of the observation group; the rate of neonatal asphyxia in the control group was 6.12%, which was slightly lower than 6.90% of the observation group, the differences were not statistically significant（P〈0.05）. There was no significant difference in Apgar score between the two groups（P〈0.05）. The asphyxia rate of vaginal delivery（5.26%） in the observation group was slightly lower than 8.16% of caesarean section, but the difference was not significant（P〈0.05）. Conclusion The most suitable delivery mode of the advanced age primipara can be chosen according to the basic state of the parturient, cesarean section is not the only choice for them. It may be appropriate to increase the vaginal delivery rate of advanced age primipara, combining with strict monitoring measures, which can reduce the adverse events of the newborn.
