彩色多普勒超声在血液透析患者自体动静脉内瘘中的应用

Clinical application of color Doppler ultrasonography in autogenous arteriovenous fistula of hemodialysis patients

目的探讨彩超在血液透析自体动静脉内瘘(AVF)的临床应用方法与价值。方法选择在我院诊治的终末期肾病患者170例作为研究对象,患者均进行了长期血液透析,采用彩色多普勒超声诊断仪分析AVF情况。结果在170例患者中,AVF通畅104例,66例出现各种并发症,并发症总发生率为38.8%。其中血栓形成34例,内瘘狭窄10例,头静脉瘤样扩张10例,血肿形成6例,盗血综合症6例。AVF通畅患者的桡动脉、吻合口PSV均明显高于AVF并发症患者(P<0.05),不同患者的RI比较,差异不显著(P>0.05)。结论彩色多普勒超声可以有效判断血液透析AVF通畅情况,有利于早期发现内瘘并发症,是临床诊断内瘘状况的有效手段。

Objective To explore the clinical application and value of color Doppler ultrasonography in autologous arteriovenous fistula（AVF） of hemodialysis patients. Methods A total of 170 patients with end-stage renal disease diagnosed and treated in our hospital were selected as the research objects, and long-term hemodialysis was performed. The AVF was analyzed by color Doppler diagnostic apparatus. Results In 170 patients, AVF patency in 104 cases, 66 cases were occurred various complications, the total incidence of complications was 38.8%, of which thrombosis in 34 cases, fistula stenosis in 10 cases, head vein tumor expansion in 10 cases, hematoma formation in 6 cases, stolen blood syndrome in 6 cases. Radial artery, anastomotic PSV in patients with AVF patency were significantly higher than those in patients with AVF complications（P〈0.05）. There was no significant difference in RI between patients with AVF patency and AVF complications（P 0.05）. Conclusion Color Doppler ultrasonography can effectively judge the patency of AVF in hemodialysis patients,which is helpful for early detection of fistula complication, which is an effective method for clinical diagnosis of fistula.
