心脉隆注射液联合IABP泵治疗急性心肌梗死心源性休克的临床研究 被引量:6

Clinical Observation on Xinmailong Injection and IABP in the Treatment of Acute Myocardial Infarction Complicated with Cardiac Shock

摘要 目的评价静脉使用心脉隆注射液联合主动脉球囊反搏泵(IABP)治疗急性心肌梗死(AMI)合并心源性休克的疗效。方法将100例急性心肌梗死合并心源性休克的病人按随机数字表法随机分为治疗组和对照组,每组50例。两组都给予IABP泵扩血管等对症治疗。治疗组静脉给予心脉隆注射液5mg/kg,2次/日,治疗两个疗程共10d。比较两组病人左室射血分数(LVEF)、血清脑利钠肽(BNP)浓度,按照美国纽约心脏病协会(NYHA)心功能分级标准评价两组心功能疗效。结果治疗组治疗后LVEF较治疗前提升(P<0.05),且高于对照组治疗后(P<0.05);两组治疗后血清BNP水平较治疗前明显下降(P<0.05),但治疗组下降更明显(P<0.05)。治疗组总有效率为88.0%,高于对照组的72.0%,差异有统计学意义(P<0.05)。结论心脉隆注射液联合IABP泵治疗急性心肌梗死心源性休克疗效显著,能有效改善临床症状。 Objective To observe the effect of Xinmailong injection(XMLI)and intra-aortic balloon pump(IABP)in the treatment of acute myocardial infarction(AMI)complicating cardiac shock.Methods One hundred patients with AMI complicating cardiac shock were randomly divided into two groups:control group(n =50)treated with IABP and routine treatment,and treatment group(n =50)treated with XMLI plus IABP and routine treatment for 10 days.The left ventricular ejection fraction(LVEF)and serum brain natriuretic peptide(BNP)concentrations were compared between the two groups,and the cardiac function was evaluated before and after treatment according to the New York Heart Association(NYHA).Results The LVEF obviously increased in treatment group after treatment,which was better than that in control group(P 〈0.05).The concentrations of serum BNP decreased in two groups after treatment,which was lower in treatment group than that in control group(P 〈0.05).The total clinical effective rate was significantly higher in treatment group than that in control group(88.0% vs 72.0%,P 〈0.05).Conclusion XMLI and IABP can effectively improve the symptoms in patients with AMI complicating cardiac shock.
